About this file

This is an Addendum No. 1 to a Request for Qualifications (RFQ) issued by the City of Greenville, North Carolina (Engineering Department) for professional consulting services on the Lake Ellsworth Project (RFQ 25-26-34). The project requires development of an Emergency Action Plan (EAP) and downstream inundation maps for the Lake Ellsworth dam, a Class-C high hazard structure. Qualifications submissions are due by Thursday, February 26, 2026, at 4:00 PM at the City of Greenville Engineering Department (201 W. Fifth Street, Greenville, NC 27858), directed to Michael Turner, with submissions accepted via sealed envelope or Dropbox link. The addendum addresses eleven questions and clarifications submitted by prospective consultants, providing critical information about the project scope, existing documentation, and site conditions. Key responses indicate that the City has no as-built drawings, design drawings, or topographic/bathymetric survey records for the dam; inspections have not been performed on the dam structure itself, though CCTV camera inspections exist for the primary and emergency spillway outlets showing degraded pipes; hydrologic modeling may be included in the scope with consultants expected to demonstrate modeling capabilities; and the dam's Class-C high hazard status was determined by the North Carolina Department of Environmental and Management of Lands and Resources (DEMLR) based on the presence of homes and roads downstream within potential breach inundation zones.

The addendum clarifies that under North Carolina law, workers' compensation insurance is only required for businesses with three or more employees, establishing an exemption for sole proprietorships and very small firms. The City indicated that solicitation documents are available via link on the procurement page and that the Watershed Master Plan information regarding the dam's hydrologic and hydrologic modeling basis is publicly available. The addendum does not address pricing, incumbents, funding sources, or contract values, as these details are reserved for subsequent negotiation following consultant selection anticipated in March 2026 with contract award in April 2026.

Questions and RFI submitted for discussion by email:

1. My understanding is that, under North Carolina statute, firms without employees are exempt from the workers’ compensation and employer's liability insurance requirement. I wanted to confirm whether this exemption would be acceptable to the City for this RFQ, or if any alternative documentation is required.?

Answer: Under NC law, workers compensation is only required when business has three or more employees.

2. I was searching on the city’s procurement page for the solicitation documents for the Lake Ellsworth Project, but they don’t appear to be listed anywhere. Would you be able to forward them to me?

Answer: Yes. Link provided. Bid Postings • Lake Ellsworth Project

3. When was the Lake Ellsworth dam constructed, and have any modifications, improvements, etc. been made to the dam since its original construction?

Answer: This information is unknown.

4. Does the City have as built/record drawings, design drawings, or other pertinent engineering design documents for the dam?

Answer: No.

5. Has a topographic and/or bathymetric survey been performed for the dam embankment and/or reservoir?

Answer: The City has no records of these surveys being conducted.

6. Has the City or another party performed a condition inspection of the dam and appurtenances and developed a report?

Answer: No. Inspections of the dam itself have not been performed. CCTV camera inspections have been conducted on the primary outlet and emergency bypass outlets. The City has the records of the camera footage.

7. Are there any known condition issues or concerns with the dam?

Answer: The existing primary spillway and emergency spillway pipes are degraded per the CCTV report.

8. Has the dam been previously studied by hydrologic and hydrologic modeling or other methods? More specifically, what was the basis for determination of the dam’s Class C (high hazard) status?

Answer: High level modeling occurred during the Greens Mill Run Watershed Master Plan process. This information is available at Watershed Master Plans | Greenville, NC. DEMLR, per the Jurisdictional

C O G D O C # 1 1 8 4 4 6 1 v 2 9 P a g e 2 | 2

Determination and Hazard classification letter, has determined that a Class-C, High Hazard classification is presently warranted due to the presence of homes and roads downstream from the dam within the potential breach inundation zone(s).

9. Does the RFQ scope of services include utilizing existing modeling, or preparing new models from scratch to develop inundation mapping?

Answer: Modeling may be included as part of this project. Consultants should provide qualifications for modeling abilities.

10. Has the City previously submitted any documentation, studies, etc. for the dam to NC Dam Safety?

Answer: No.

11. Is the City able to provide any of the aforementioned documentation to interested consultants in advance of the RFQ submission deadline?

Answer: Not applicable.
