Brief description of services to be provided:

BACKGROUND:

The State of Michigan (hereinafter referred to as “State”) is under Contract 230000000988 with Agate Software Inc (“Contractor”) to provide Hosting, Maintenance and Operations support for the Grant Management System titled IntelliGrants™ IGX. This Statement of Work (“SOW”) amends the MDHHS use of the IntelliGrants™ IGX software and the support services as described in the Contract Change Notice (CN) 2. Unless otherwise identified within this SOW, all Contract Terms and Conditions remain in full effect.

PROJECT OBJECTIVE:

The Division of Victim Services (DVS) sees a need to track additional information from the courts on collection of these fees. DVS needs for this monthly data to be reconciled not only by the total dollar amount of fees, but to include in this data in line-item detail of each defendant ordered to pay into the fund each month. Inclusion of this data will eliminate the highly labor-intensive work of manually reconciling partial payment data, increasing program accuracy, and reducing time on these efforts to allow staff to focus on their primary duties.

This system will provide the following:

• Automate data entry based on user profiles.

• Ensure required fields are completed for first-time submissions.

• Support multiple victims, defendants, and payments per case.

• Enable approval workflows and notifications.

• Generate reports and letters based on payment status.

• Handle both claimed compensation and unclaimed restitution scenarios.

Version 2026-1 2 of 4

SCOPE OF WORK:

The following information outlines the project scope assumptions that apply to the requested enhancements to the electronic grants management system. This document serves as the framework for the configuration of the IntelliGrants™ IGX system.

1. Parent Design

a. Form Fields: Includes fields like County, Court Name, Case Number, Crime

Type, Defendant

• Name, Claimant/Victim Name, Amount Paid, Restitution Ordered

Amount, etc.

b. Logic & Validation:

• Auto-population of fields after first submission.

• Mandatory fields for first-time case entries.

• Conditional visibility (e.g., “Crime Type Other” appears only if

“Other” is selected).

c. Workflow:

• Approval process for submitted payments.

• Status tracking (Submitted, Pending, Accepted, Rejected).

• Email notifications for rejected payments.

• Role-based view/edit permissions.

2. Child Design

a. Mirrors the Parent Design but tailored for child records or linked cases.

b. Includes additional fields like “Restitution Owed to” and “Compensation

Balance”.

c. Supports cascading dropdowns based on crime type.

d. Emphasizes data consistency and auto-calculation of balances.

5. Child (Detailed Design)

a. Adds context for dropdown categories:

b. Unclaimed Restitution: e.g., Check Fraud, Retail Fraud, DUI without injury.

c. Crime Victims Compensation: e.g., Assault, Arson with injury, Sexual

Assault.

d. Includes capabilities for:

e. Bulk upload by courts.

f. Data export by DVS staff.

g. Letter generation for various payment scenarios (e.g., overpayment, bounced checks).

Version 2026-1 3 of 4

h. Report generation (e.g., by date range, court, defendant).

6. Reports

a. Payment Details Report: Based on fields 1–17, searchable by case, defendant, victim, and date.

b. Raw Data Report: Comprehensive search criteria including court name, case number, payment date, etc.

Brief description of services to be provided:

BACKGROUND:

This Statement of Work (SOW) is part of a Change Notice to Contract No. 230000000988 (Contract) between the State of Michigan (State or SOM) and Agate (“Contractor” or “Vendor”) under the terms of the Implementation Services Agreement (Contract # 230000000988) (“Agreement” or “Contract”). The Parties agree as follows:

The Michigan Department of Agriculture and Rural Development offers this grant opportunity to county fairs. This program will assist in the promotion of building improvements or other capital improvements on county fairgrounds within the state.

Building improvements are costs related to making changes to improve buildings which are used for fair purposes and are owned by the fair association or political subdivision. These improvements will extend the useful life of the building.

Capital improvements are costs related to making changes to improve capital assets, increase their useful life, or add to the value of these assets. Capital improvements may be structural improvements or other renovations to a building or enhance usefulness or productivity.

The goal of the Forestry Assistance Program (FAP) is to help family forest owners achieve their goals for woodland ownership. The program provides outreach and technical assistance to help private landowners make good decisions about how to care for their land. A survey conducted by Michigan State University of family forest owners concludes that only 20% of the 11-million-acre resource is actively managed. The Forestry Assistance Program works with local conservation districts to increase the number of family forests that are managed under a sustainable forest management plan.

PROJECT OBJECTIVE:

The objective of the project is to develop a new grants system to help in managing MDARD grant programs from Request for Proposal to closing out a grant award. The system will help manage applicants, grant and grantee information. The system will help with data management, financial review assistance, and active grant monitoring.

The system will support the development of an electronic grant application submission, competitive application review process, and management of awarded grant projects.

SCOPE OF WORK:

Version 2 (3/2021) 2

Extend the project from Dec 9, 2025, out to June 30, 2026 to complete all the Grant application workflow pages.
