About this file
This document is a comprehensive insurance policy package for Anthony Wayne Rehabilitation Center (A.W. Holdings, LLC d/b/a Benchmark Human Services), providing various insurance coverages through Philadelphia Insurance Company and Starr Indemnity & Liability Company. The policies specifically name the State of Arizona, Department of Economic Security, and its departments, agencies, boards, commissions, universities, officers, officials, agents, and employees as additional insureds. The coverage includes commercial general liability, automobile liability, workers' compensation, professional liability, and other specialized endorsements effective from July 1, 2025 to July 1, 2026.
The insurance policies provide coverage with substantial liability limits, including $1,000,000 per occurrence for commercial general liability, $3,000,000 aggregate, and additional specialized coverages such as abuse and molestation liability ($1,000,000 per incident, $3,000,000 aggregate), professional liability, and workers' compensation. The policies are associated with a specific contract (Contract ID: CTR077671) for the Division of Aging and Disability Services (ADES), and include multiple endorsements that provide waivers of subrogation, additional insured status, and primary non-contributory insurance provisions. The total premium for the workers' compensation policy is $546,513.00, with the policies designed to protect the insured's operations and the state's interests across various potential liability scenarios.
