Healthcare Interoperability Resources (FHIR), maintenance and enhancement of associated vocabularies, toolsets and business processes in support of the standards-based reporting to the National Healthcare Safety Network

(NHSN).

B.2 Indefinite-Delivery/Indefinite Quantity Contract

The total MAXIMUM estimated cost to the Government for performance of the contract requirements for 12 months shall not exceed the TO BE ADDED AT TIME OF AWARD. The MINIMUM dollar obligation of the

Government under this contract is $1,000. Obligation of funds will be done by the issuance of individual task orders by the Contracting Officer in accordance with the clause titled “Issuance of Task Orders” in Section H and FAR Clause 52.216-18 titled “Ordering” in Section I. Task orders will be issued on a Firm Fixed Price (FFP), Time and

Materials (T&M), and Labor-Hour (LH) or a combination thereof, as determined to be in the best interest of the Government. The Contractor shall not exceed the amount negotiated for each individual task order without prior written approval of the Contracting Officer.

In addition, the Contractor shall not commence work until a task order or other written notification for a specific assignment is issued by the Contracting Officer. Only a CDC Contracting Officer is authorized to issue a task request to the Contractor or issue finalized task orders under this contract. Contracting Officer.

Telephone or other verbal task orders are not authorized under this contract unless issued by a duly warranted

Government Contracting Officer on an emergency basis.

B.3 Performance-Based Acquisition

This is a performance-based acquisition. The performance-based acquisition strategy adopts the best commercial practices and provides the means to reach world class commercial suppliers, gain greater access to technological innovations, maximize competition and obtain the best value to achieve greater savings and efficiencies. Use of performance-based contracting strategies reduces acquisition cycle time and costs since contractors are not compelled to perform to detailed design-type specifications that inhibit creativity and efficiency.

B.4 Non-Personal Services Contract

This is a non-personal services contract as defined in accordance with Federal Acquisition Regulation (FAR)

37.101. The Government will evaluate the quality of support services provided, but the Contractor retains control over its employees or agents. B.7 Minimum Ordering Amount

This contract guarantees that task orders amounting to a minimum of one thousand dollars ($1,000) will be issued during the base period ONLY for those contractors who submit a minimum of two (2) proposals for task orders during that period. In the event that during the base period, the contractor receives obligations of less than the $1000.00 minimum, the Government will fund the difference between the actual obligation and the guaranteed minimum. This obligation is satisfied when the contractor is issued a task order for at least $1000.00 on or before the end of the base period. There is no guaranteed minimum for the option periods.

B.8 Maximum Ordering Amount

The maximum amount of all orders issued and awarded under all contracts over the five (5) year period of performance shall not exceed $15,000,000.00.

The maximum ceiling amount of this contract may be increased to accommodate additional needs of the Agency that are within the scope of the contract.

(End of Clause)

B.9 Ordering Procedures

The Ordering Procedures shall be in accordance with FAR 16.505 titled Ordering. Individual task orders will clearly describe all of the requested services to be performed so the full price proposed can be established when the order is placed. Orders shall be within scope, issued with a period of performance, and be within the maximum value of the contract.

Pricing to the Government for the services provided shall be determined for each task order based on the established fixed hourly rates and the scope of work.

The procedures and selection criteria that the Government will use to provide awardees a fair opportunity to be considered for each order (FAR 16.505(b)(1)).

Section C - Description/Specification/Work Statement

STATEMENT OF WORK

Development and Maintenance of Clinical Data Architecture (CDA) Implementation Guides, Vocabularies, and Related Toolsets for Healthcare Associated Infection (HAI) Reporting to the National Healthcare Safety Network

Background and Need

The National Healthcare Safety Network (NHSN) is a government-owned web-based information system designed for surveillance of patient and healthcare worker safety, antimicrobial use and resistance, adherence to prevention practices and extent to which core elements of antimicrobial stewardship are used. The principal goals of the NHSN are to improve safety, adherence to prevention practices, and use of antimicrobial stewardship by aggregating, analyzing, providing, and summarizing data for action. The public health importance of healthcare-associated infections (HAIs) has prompted an increasing number of States to enact laws requiring hospitals and/or other healthcare facilities to report HAI data to the health department or another State agency. In 33 States that have enacted mandatory reporting requirements as of December 2015, state statutes or implementation plans call for hospitals and/or other healthcare facilities to report HAI data to the health department or another State agency using the NHSN. Further, beginning in 2011, the Centers for Medicare and Medicaid Services (CMS) has required healthcare facilities to submit HAI and other data to NHSN to qualify for financial incentives and/or avoid penalties.

As a result of state mandates and CMS requirements, the number of healthcare facilities participating in NHSN has increased exponentially since the system was launched in 2005. Increasing use of NHSN by long term care facilities and ambulatory surgery centers is on the horizon along with increasing participation by all healthcare facilities in

NHSN antimicrobial use and resistance surveillance. The rapid growth of NHSN and the mounting number of system users and uses place a premium on maximizing the use of healthcare data in electronic form for NHSN’s surveillance purposes, leveraging prevailing healthcare data interoperability standards, i.e., Clinical Document

Architecture (CDA) and Fast Healthcare Interoperability Resources (FHIR), assuring that data submitted to NHSN via electronic messages are successfully merged and analyzed with NHSN data submitted manually, and integrating efforts to expand antimicrobial use and resistance reporting to NHSN with complementary efforts, such as electronic laboratory reporting of communicable diseases data to state and local health departments.

Purpose

The Division of Healthcare Quality Promotion (DHQP) requires an indefinite delivery, indefinite quantity (IDIQ) contract to continue and expand with the development and the maintenance of the existing CDA implementation guides, new use of Fast Healthcare Interoperability Resources (FHIR), maintenance and enhancement of associated vocabularies, toolsets and business processes in support of standards-based reporting to NHSN, and a focus on antimicrobial use and resistance surveillance via electronic messaging from healthcare facilities to NHSN.

Project Objective

The purpose of this requirement is to acquire an IDIQ contract to provide a mechanism with rapid turnaround to obtain support services related to program operations or public health activities in priority areas. Some general objectives of this effort shall include:

a. Ensure that CDC has ongoing timely, professional and scientific services which leverage industry best practices and professional standards;

b. Ensure related and interdependent functions and disciplines are covered in the contract services;

c. Provision of cost effective solution-based services

d. Support the achievement of DHQP’s mission and program goals.

Scope of Work

Independently, and not as an agent of the government, the Contractor shall furnish all the necessary personnel, facilities, supplies, and equipment required to perform services and deliverables as described within individual task orders.

Technical Requirements

The Contractor shall provide contractor personnel to fulfill the Statement of Work requirements listed under each Task Area. The categories and tasks listed are a general description of the type of services that may be requested.

The specific scope, technical requirements, and deliverables/schedules will be described in each request for task order proposal issued. All tasks shall fall within one or more of the task areas:

a. HL7 Implementation Guidance Development and Maintenance

Provide support in the continued expansion and enhancement of the existing Healthcare Associated Infection (HAI HL7 Implementation Guide for CDA ® Release 2—Level 3 or most recent Normative version) Healthcare Associated Infection Implementation Guides(IGs). For new HAI definitions not yet incorporated into existing IGs, also create a corresponding Fast Healthcare Interoperability Resources (FHIR) resource bundle. When appropriate, assist in the balloting process for these new FHIR profiles.

Also when appropriate, generate FHIR profiles for all existing HAI event types both numerator and denominator. Appropriate timeliness is to be interpreted as either a desire on the part of CDC to implement FHIR in a production or pilot environment or when the FHIR specification moves from the existing Draft

Standard for Trial Use (DSTU) to Normative status or at the discretion of CDC but within the scope of the award.

b. Vocabulary Development and Maintenance

Provide a full-time contractor personnel for the development and eminence for all of the NHSN vocabulary needs.

c. Provide Statistical and Analytics Support for the Analysis of NHSN Data

Conduct an analysis of data submitted to the NHSN via electronic files (eFiles) and Manual Data Entry

d. Integration of Antimicrobial Resistance Surveillance and Electronic Laboratory Reporting

Provide an Information Technology and Informatics subject matter expertise to further integrate antimicrobial resistance surveillance to NHSN with electronic reporting of communicable disease data.

Reporting Schedule

In addition to specific reporting requirements defined in each Task Order issued, the Contractor shall furnish monthly progress reports detailing current status of each Task Order currently awarded under the contract. The report shall be narrative in form and shall include a summary of progress toward completion of each Task Order and any problems encountered to date, including the Contractor’s assessment of the specific impact of such problems on scheduled date of completion of milestones.

Government Furnished Property

Each task order will list the place of performance. In the task orders where the place of performance will be at the Government’s facility, the government will provide the required workspace and equipment. Any additional government property to be furnished will be specified in each task order.

The order shall be specifically identified as a stop-work order issued under this clause. Upon receipt of the order, the Contractor shall immediately comply with its terms and take all reasonable steps to minimize the incurrence of costs allocable to the work covered by the order during the period of work stoppage. G.5 CDC42.0002 Evaluation of Contractor Performance Utilizing CPARS (April 2013)

In accordance with FAR 42.15, the Centers for Disease Control and Prevention (CDC) will review and evaluate contract performance. FAR 42.1502 and 42.1503 requires agencies to prepare evaluations of contractor performance and submit them to the Past Performance Information Retrieval System (PPIRS). The CDC utilizes the

Department of Defense (DOD) web-based Contractor Performance Assessment Reporting System (CPARS) to prepare and report these contractor performance evaluations. All information contained in these assessments may be used by the Government, within the limitations of FAR 42.15, for future source selections in accordance with FAR

15.304 where past performance is an evaluation factor.

The CPARS system requires a contractor representative to be assigned so that the contractor has appropriate input into the performance evaluation process. The CPARS contractor representative will be given access to CPARS and will be given the opportunity to concur or not-concur with performance evaluations before the evaluations are complete. The CPARS contractor representative will also have the opportunity to add comments to performance evaluations.

The assessment is not subject to the Disputes clause of the contract, nor is it subject to appeal beyond the review and comment procedures described in the guides on the CPARS website. Refer to: www.cpars.gov for details and additional information related to CPARS, CPARS user access, how contract performance assessments are conducted, and how Contractors participate. Access and training for all persons responsible for the preparation and review of performance assessments is also available at the CPARS website.

H.2 Health Information Technology (Jan 2012)

Executive Order 13410: Promoting Quality and Efficient Health Care in Federal Government Administered or

Sponsored Health Care Programs promotes efficient delivery of quality health care through the use of health information technology, transparency regarding health care quality and price, and incentives to promote the widespread adoption of health information technology and quality of care. To support this objective the awardee shall adhere to the following interoperability standards and requirements. For purposes of this contract clause, interoperability is defined as the ability of two or more systems or components to exchange information and to use the information that has been exchanged.

The Health Information Technology Clause (January 2012) shall be inserted in Section H of all contracts that involve (1) the exchange of public health data between public health entities, CDC, and other federal agencies, or (2) the exchange of public health data within the clinical setting, such as hospitals and providers, or any combination of

(1) and (2), and covers both situations. The inclusion of this clause is a Contracting Officer decision made with the advice and in coordination with the appropriate program officials from the requesting office.

Interoperability of Health Information Technology This section of the clause applies to contracts that involve the exchange of public health data between public health entities, CDC, and other federal agencies. This supports the development and use of interoperable information systems for public health functions such as biosurveillance, situational awareness, laboratory science, or environmental health.

In addition to complying with HHS Standards for Security Configurations (HHSAR Clause 352.239-70), HHS Standards for Encryption Language (HHSAR Clause 352.239-71) and Federal Laws, Regulations and Standards related to IT Security (HHSAR Clause 352.239-72), the awardee is required to meet recognized interoperability standards as follows:

The awardee shall use nationally adopted Public Health Information Network (PHIN) standards

(www.cdc.gov/phin) at the time of the system implementation, acquisition, or upgrade, in all relevant information technology systems supported, in whole or in part, through this contract. Where no PHIN standards exist, the awardee shall utilize recognized standards established by the Office of the National

Coordinator for Health Information Technology (ONC) as described in their Nationwide Health Information Network (NwHIN) standards (www.hhs.gov/healthit/standards/background).

The awardee shall use PHIN policies, standards, practices, and services in the research and implementation of efficient and effective public health information systems, to facilitate interoperability (the ability of two or more systems or components to exchange information and to use the information that has been exchanged) with public health organizations and networks, including NwHIN.

Where the awardee supports or participates in health information or data exchange with disparate entities, the awardee must have an architecture that is compatible with the architecture of PHIN, where appropriate.

If PHIN standards do not exist, the awardee shall refer to NwHIN standards (www.hhs.gov/healthit/standards/background).

Public Health Data within Clinical Healthcare This section of the clause applies to contracts that involve the exchange of public health data within the clinical setting, such as hospitals and providers. This supports the exchange of public health information, including laboratory exchange data, between public health entities and healthcare systems.

The awardee shall use health IT investments, such as electronic health records (EHRs), personalized health records, and network components through which they operate and share information, that are certified by a recognized certification board and/or validated and tested with PHIN (www.cdc.gov/phin) or NwHIN (http://healthit.hhs.gov/portal/server.pt?open=512&objID=1153&mode=2) certification to ensure a minimum level of interoperability or compatibility of health IT products.

The awardee may use services available from the Agency for Healthcare Research and Quality (AHRQ) at www.healthit.ahrq.gov. This link provides technical assistance, identifies challenges to health IT adoption and use, and identifies solutions and best practices that have the potential to transform clinical practice through the best and most effective use of IT.

Where feasible, the awardee shall collaborate with other healthcare entities, local quality improvement organizations, and/or local medical societies to promote the adoption of certified EHRs in clinical provider settings.

Where feasible, the awardee shall collaborate with other healthcare entities and/or local hospital associations to promote the adoption of certified EHRs in hospital settings.

The awardee shall identify public health entities that have a stake in the award and demonstrate interactions with them to implement and promote the adoption of PHIN and/or NwHIN data exchange standards in support of federal health initiatives (e.g., situational awareness, EHR Meaningful Use).

The awardee shall report on public health information exchange activities that include partnerships with health information exchanges, other public health partners, or any other stakeholders within program reports, progress reports, or as requested by CDC.
