Primary conditions - must be consistent with World Trade Center exposures Secondary conditions - must be as a result of a primary condition.

Work-up - diagnotic testing that must be associated with a suspected primary condition Parameter of Use - guidance for when a secondary code or work-up code is used

“Complications from Treatment” – 1st problem drug class are the corticosteroids and list the following:

1 For patients who have been prescribed oral corticosteroids for a WTC covered condition by a WTC Program Physician for 12 months or more shall have standard of care monitoring/treatment covered by the WTC Program for osteoporosis (includes baseline [if needed] and serial bone density scans and then as indicated pharmaceutical therapy).

2 For patients who have been prescribed oral corticosteroids for a WTC covered condition by a WTC Program Physician for 12 months, treatment of non-traumatic fractures will be covered if there is evidence of osteoporosis in the involved bone.

3 For patients who have been prescribed oral corticosteroids by a WTC Program Physician (independent of duration, as this is idosyncratic), diagnosis/treatment of aseptic necrosis of hips or other joints will be covered.

4 For patients on oral corticosteroids for a WTC covered condition who then develop new-onset diabetes and/or new-onset hypertension, their physician shall make every reasonable attempt to safely reduce/discontinue oral corticosteroids as clinically indicated. If despite that effort, the new-onset diabetes and/or new-onset hypertension persists, then therapy of new-onset diabetes and/or new-onset hypertension is covered as a secondary condition under the WTC Program. For now exacerbations of prior diabetes and/or hypertension is not a covered secondary condition.
