Pulmonary Function Tests

The NRHP contractor shall contract with providers to perform spirometry testing according to CPT code 94010 and the following quality requirements:

• The spirometer must be configured to print numeric results, flow-volume curves, and volume-time curves from the three best maneuvers (not just the single best).

• The spirometer must be configured to use NHANES III (Hankinson 1999) reference equations.

• A record showing the results of a 3.00 liter calibration check, done on the same day as patient testing, must also be provided.

• The name of the technologist performing the test must be written on the test report.

• The technologist must have successfully completed an approved course in spirometry

(consistent with applicable sections of 29CFR1910).

• Spirometry tests which do not meet American Thoracic Society standards for 3 acceptable and 2 repeatable maneuvers will not be reimbursed by this program.

• Any post-bronchodilation testing will be performed in conjunction with treatment services and is not part of the monitoring exam.

PFTs with FEV1/FVC variance up to 200ml may be accepted. PFTs that fall into the 150-200ml category will be acceptable for evaluation & exam purposes. An ideal result should be <150ml to meet ATS protocol & be acceptable for epidemiologic purposes.

Each month the NRHP contractor shall select a random sampling of PFT results (10% of the tests performed during the month) and send them to a NIOSH designated physician for a Quality Assurance Review.
