Vendor Responsibility Questionnaire Summary

This is a Vendor Responsibility Questionnaire (VRQ) document established by the State of Georgia to evaluate vendor integrity and procurement risk for state and local contracts. The VRQ is required for all formal solicitations exceeding $100,000 and those involving public health, infrastructure, or operational service delivery. The questionnaire is designed to ensure compliance with Georgia procurement laws, including O.C.G.A. 50-5-50, 50-5-82, and 50-5-84 series, which mandate evaluation of bidder responsibility, prohibition of contracts with vendors having unpaid taxes or regulatory violations, and restrictions on contracting with scrutinized companies or foreign companies of concern. The VRQ requires vendors to complete all sections truthfully, provide detailed explanations for affirmative responses in the Legal & Regulatory Compliance section, and obtain notarization of the final page with valid commission expiration dates.

The questionnaire comprises five primary sections addressing general vendor identification, legal and regulatory compliance, financial and operational stability, tax compliance, and certification. Vendors must disclose information regarding debarment or suspension history, environmental law violations, OSHA citations, procurement fraud convictions, foreign company status, contract completion failures, ongoing investigations, financial capacity, insurance maintenance, payment and performance bond ability, and tax standing with the Georgia Department of Revenue. All fields are mandatory unless marked as not applicable, and any affirmative responses require supporting documentation. The authorized signatory must certify under oath that provided information is accurate and complete, with understanding that false disclosure or omission may result in immediate disqualification or contract termination.
