The solicitation number is BPM006769 and specifically relates to Parenting Time Services. The form is focused on compliance with Arizona Revised Statutes (A.R.S.) § 35-394, which requires companies contracting with public entities to certify they do not use forced labor of ethnic Uyghurs in the People's Republic of China.
The document is completed by StarrCare In-Home Services, a company located in Tucson, Arizona, with contact information including an email (tstarr@starrcare.org) and phone number (520) 901-0117. The company has selected the checkbox indicating they do not use forced labor of ethnic Uyghurs, any goods or services produced by such labor, or work with contractors, subcontractors, or suppliers that engage in such practices. The form provides exemptions for sole proprietorships, companies with fewer than ten employees, and non-profit organizations, though StarrCare does not appear to qualify for these exemptions.
