In 2017, we expanded our services by obtaining the Supervised Visitation contract with ADCS to reunify children in DCS custody with their families. LifeTran Supervised Visitation Program served children of all ages, their parents, and other caregivers, who have histories of child abuse, severe neglect, domestic violence, substance abuse and/or mental illness.
We have solidly made our mark throughout the thirteen (13) counties we currently serve. Our hardworking and dedicated team have worked extremely hard to ensure that the families we serve are provided with top-notch services. We have gone above and beyond to meet the needs of the families we serve, and we have answered every ADCS call to service whenever our services are needed.
In 2020, in our quest to continue to diligently serve the families in the rural and urban communities involved in ADCS Case across the State of Arizona and bridging the gap in service barriers to these families, we obtained the Clinical Supervised Parenting Time contract with ADCS.
This service was contracted to two (2) providers and LifeTran being the second provider.
However, the latter withdrew leaving LifeTran to take up the mantle of being the sole provider of these services across Arizona. Despite facing similar challenges as other providers, we never falter; rather, we will continue to stand strong in our partnership with ADCS to deliver the much-needed services to the community we serve.
Already, we are making our mark in this field through our effective and on-time service delivery, efficient communication between ADCS stakeholders, and our dedicated team of professionally qualified Clinicians and Counselors. We have built trust amongst our clientele, and we are known for our compassionate approach and understanding when dealing with our unique clients from a different spectrum of life.
Furthermore, in 2020 also, LifeTran secured a contract to provide on-site nursing services at the
State of Arizona eleven (11) Intermediate Care Facilities for Individuals with Intellectual
Disabilities (ICF/IID) AND nine (9) State Operated Group Homes (SOGH) in Maricopa and/or

Services include providing quality health care, supporting habilitative and active treatment services to eligible individuals (DDD Members) served by the Division of Developmental
Disabilities. The various layers of these services are performed by our best-in-class Registered
Nurses (RN) and Licensed Practical Nurses (LPN).
Through the addition of the Clinical Supervised Parenting Time and Nursing services, we have reached our long-term double goals of not only providing diverse health and social pool of services but doing so by serving communities across all the fifteen (15) counties in the State of Arizona.
This feat has solidified our presence in all rural and urban communities in the state with the addition of the Family Connections, Nurturing Parenting Program, and SENSE services.
We have the skillset in Parenting Time services and our team of professional staff who are already serving in this role and meets the Scope of Work requirements set forth by ADCS to hit the ground running from day one in serving families that are referred to LifeTran.
Working in collaboration with ADCS and courts, LifeTran accepts referrals from ADCS who determine that parent-child interaction must be supervised to ensure the children’s safety. Rather than eliminating contact between the parent and child, which can be highly detrimental to the child, our partnership with ADCS enabled us to supervise parenting time with the families, providing them with a one-on-one supportive setting where their relationship can safely continue while we also provide and address other necessary services that the parents may need, such as substance abuse or mental health treatment, counselling, family, and court matters.
We started the supervised visitation services with the vision to use our services and well trained/diverse employees to strengthen family relationships, through visitation and provide a child focused environment for families in transition.
In the past five (5) years, the performance in our program has proven effective to list a few as evidenced by.

• Effective communication, professional insights, and deepened engagements with ADCS team of Case Specialists/Supervisors, Fidelity and Compliance have been centerpiece to our success.
• We have met all the timeframes set forth by ADCS for better continuity of care to our clients.
• We have a combined total of thirty (30) conducive rooms/facilities, with state-of-the-art amenities and security to support the children and their families during visitation.
• We have expanded from initially serving seven (7) Arizona counties, then serving Eleven

In the years we have served the community providing Parenting Time Services, our skilled and professional staff members have handled first-hand cases with clients with ventilators, parenteral nutrition support, and continuous oxygen as well as dialysis machine.
LifeTran Parenting Time services is designed to help sustain healthy relationship between parents and their children affected by history of child physical or sexual abuse, domestic violence, substance abuse, or mental illness.
Our assigned families will receive support from skilled clinicians/professionals with vast experience in social and human services to strengthen or repair the parent’s relationship with their children. LifeTran will not only maintain safety during the sessions but will also help the parents referred to us to repair and strengthen the ruptured bonds between them and their children due to trauma and separation, improve parenting skills and engagement, and reduce the risk of future abuse or neglect.
Though our office is traditional, our staff work non-traditional hours to meet the needs and schedule of our clients as demands. We also take great joy in mentioning that we have been diligently serving our Rural and Regional communities in Arizona since inception of our partnership with ADCS and we have delivered immensely, oftentimes exceeding expectations.
It is common knowledge that servicing rural communities comes with its enormous challenges ranging from but not limited to distance, lack of resources, and dearth of quality/limited manpower.
However, LifeTran team, having already deepened its foothold providing outstanding services to these rural areas these past years, will step in from DAY ONE, to cover these areas with resources and know-how and thus help alleviate some of the challenges currently faced as our staff members take enormous pride in serving these areas.

Question 2: Provide a detailed plan of how your agency will be ready to serve cases on the go-live date, after award. The response shall at a minimum describe and provide a response for each of the following bullet points listed below:
▪ Provide the anticipated number of referrals your agency expects to have the capacity to serve on the first day of contract implementation.
▪ Does your agency anticipate an increase in capacity throughout the contract term? If yes, describe your plan for scaling up, including timelines, staffing, and resource allocation.
▪ Describe how your agency tracks and manages service capacity and the progression of referrals (cases) in real-time. What tools, systems, or processes do you use to ensure timely scheduling and service delivery?
A. Anticipated number of referrals we will be serving and our capacity available on the first day of contract implementation?

State of Arizona
Department of Child Safety
Office of Procurement & Contracts
LifeTran is already providing services in all the counties in Arizona. Furthermore, LifeTran has consistently exceeded its contractual capacity by over 100%. For instance, LifeTran currently has a contractual capacity of 15 in Cochise County, however, LifeTran is currently servicing 40 referrals in Cochise County. Also, although LifeTran has a contractual capacity of 25 in Maricopa
County, LifeTran is currently servicing 60 referrals in Maricopa. With the track record of always going above and beyond contractual capacity requirements, and since LifeTran is already serving
850+ families on a yearly basis, logging over 15,000 service hours, LifeTran is positioned to continue serving the families under the Parenting Time Service not just on the go-live date, but several years later, a claim that LifeTran’s track record can validate.
Consequently, LifeTran will serve up to three-hundred and fifty-five (355) Parenting Time
Service referrals on the go-live date and will continue to maintain and exceed this number as
LifeTran has consistently done in the past. LifeTran currently has approximately 50 qualified personnel to dedicate to the Parenting Time service and is on track to hire and train 20 additional staff across all counties before the go-live date. These additional staff will enable LifeTran to meet and exceed the contractual capacity in all the regions.
B. Does your agency anticipate an increase in capacity throughout the contract term? If yes, describe your plan for scaling up, including timelines, staffing, and resource allocation.
The anticipated growth in capacity for LifeTran is mainly a function of the needs of ADCS. LifeTran has always risen to the opportunity to increase its capacity to meet the current needs of ADCS.
For instance, as previously mentioned, LifeTran doubled its contractual capacity in various counties due to the needs of ADCS and has continued to grow since.
Historically, the capacity needs of the Parenting Time Service have consistently grown over the years. Therefore, LifeTran has positioned itself to accept as many referrals as possible, well beyond its contractual limits, as it is LifeTran’s mission to serve and re-unify as many families as possible.

LifeTran anticipates a consistent growth in capacity throughout the contract term and has already put mechanisms in place to help with the a graceful increase in capacity, including a seamless onboarding process for new hires, a streamlined training process to quickly equip new hires with the knowledge they need to succeed while working with our clients, an in-house case scheduling platform that send reminders to case aides regarding their upcoming visitation cases and much more.
LifeTran is therefore positioned to increase its capacity at a steady pace of at least 20% per year and has established various structures and mechanisms to ensure graceful growth without sacrificing the quality of the provided services.
C. Describe how your agency tracks and manages service capacity and the progression of referrals (cases) in real-time. What tools, systems, or processes do you use to ensure timely scheduling and service delivery?
LifeTran maintains adequate full-time/part-time professionals, who have the right values and skills, to always deliver high quality services to our clients and meet/exceed our contracted capacity. We evaluate our staffing needs on a weekly basis and compare notes with the ADCS weekly waiting-list report. This enables us to hire more professionals based on the needs of
ADCS. However, to always maintain the right number of staff, it is necessary for LifeTran to track/monitor the capacity of referrals on a real-time basis. LifeTran has met this requirement by using its in-house service management system, BrainBox.
One of the key inputs into BrainBox when a new referral is received is the referral start and end dates. Another user added component is the status of a case, which can move from an active to a closed case should such a case be approved for closure. Therefore, using the referral start and end dates, and the case status, which will be different for different referrals, BrainBox, with 100%

Office of Procurement & Contracts accuracy, can show the exact capacity for each region at any given time, and on a real-time basis. Without considering extensions, BrainBox can also predict what the capacity of a region will be on a monthly basis well beyond six months. As cases are nearing expiration, Program coordinators begin contacting case managers to know if a given case will be extended. The program coordinators communicate with the assigned Case Manager 30 days before the expiration date to notify them of the upcoming expiration of the case and ask for a referral extension if applicable. The data from the case managers, confirming whether a referral will be extended or not, serves as a feedback loop for BrainBox to determine the number of additional referrals that LifeTran needs to proactively request to ensure the contractual capacity is always met.
Using BrainBox, our administrative staff members can generate a report of our current capacity level for each region, with a projection of what the capacity would be in weeks or months based on the expiration date of cases and feedback obtained from case managers on expiring cases.
The ability to access real-time capacity data for each region and to project the capacity in advance enables LifeTran to proactively notify the HR team to hire more staff as needed.
Our Quality Assurance and Compliance Manager is specifically responsible for monitoring this real-time contractual capacity data to proactively initiate the process of adding more staff to our team where needed to not only meet and exceed our contractual capacity but to closely work with
ADCS to increase our capacity to help minimize or eliminate the ADCS waiting list.

Question 3: Provide examples of challenges your agency has encountered in parenting time services and how you addressed them. Include challenges with maintaining contract capacity, if applicable.
LifeTran Corporation has consistently demonstrated strength and expertise in overcoming the challenges associated with parenting time services. Our commitment to excellence and unwavering dedication to the safety and well-being of the children and families we serve have been the foundation of our operations. However, given our extensive experience, we have encountered and successfully navigated several challenges that arise in the provision of these critical services. Below are some of the key challenges we have faced and the measures we have taken to address them effectively.
✓ Safety Concerns:
Ensuring the safety of children, parents, and staff during parenting time is our highest priority. We have encountered situations where parents have become extremely irate, raising their voices, making threats, and exhibiting violent tendencies. To mitigate these concerns, we have established robust safety protocols, which include comprehensive training for our staff in de-escalation techniques to prevent situations from spiraling out of control.
Additionally, all visitors entering our facilities undergo screening procedures, and we have employed security personnel in every one of our offices and visitation center to enforce a zero-tolerance policy for aggressive behaviors, in strict accordance with DCS policies. Our staff undergoes quarterly emergency response training, ensuring they are well-equipped to handle crises swiftly and effectively. Furthermore, we have implemented quick exit strategies to ensure the safety of children and staff in case of incidents. These proactive measures have proven effective in maintaining a secure environment for all parties involved.

✓ Logistical Challenges with Transportation:
Transportation-related issues are another common challenge that can impact the success of parenting time services. There have been instances where transportation providers fail to pick up parents for scheduled visits or neglect to retrieve them after visits have concluded. To address these concerns, we proactively communicate with the assigned DCS Case Specialists ahead of time to ensure timely transportation requests and send reminders to minimize no-shows.
In cases where the assigned transportation provider does not arrive, we take an active role in reaching out to them to ensure prompt pickup. Additionally, we remain flexible with rescheduling visits when necessary due to transportation-related delays. In rare instances where DCS approves, we provide transportation assistance to facilitate visitations and minimize disruptions.
In some rare instances where a parent was left stranded after a visit and had no financial means to get home and we were unable to reach the Case Specialist or transportation provider, we had, on occasion, arranged for ride-hailing services such as Uber or Lyft to ensure the parent could return to their intended destination. These instances are infrequent, and the decision to provide this assistance has been made solely to prevent undue hardship on the parents while upholding the integrity of our services and partnership with DCS. This support has been provided at our own expense, as part of our commitment to service excellence, without expectation of reimbursement.
Our hands-on approach has significantly reduced the frequency of missed visits and has enhanced the efficiency of our service delivery.
✓ Staffing and Retention Issues:
Hiring and retaining qualified visit supervisors (Case Aides) can be challenging due to the high-stress nature of the job, irregular hours, and the emotional toll it can take on employees. Despite these challenges, LifeTran maintains a workforce of over 50 Case Aides at any given time.
However, given our statewide service coverage, we continually recruit and train full-time and part-time Case Aides and Supervisors to fill any vacancies promptly, thereby reducing burnout and workload stress among our experienced employees.

To enhance staff retention, we offer competitive pay, flexible scheduling, and clear career advancement opportunities. These measures have helped us maintain a dedicated workforce capable of providing high-quality services while minimizing the turnover that is common in this field. By prioritizing employee satisfaction and professional development, we ensure the continued excellence of our service delivery.
✓ Challenges with Obtaining Accurate Client Information:
One of the recurrent challenges we face is obtaining accurate and up-to-date contact information for clients, placements, and children. This issue can create delays in scheduling visits and disrupt service delivery. To address this challenge, we maintain constant communication with the assigned DCS Case Specialists and Supervisors to verify and update client information as needed.
We have also implemented systematic procedures to cross-check the accuracy of the data we receive and to promptly address any discrepancies. Our proactive approach ensures that we minimize administrative delays and provide efficient and effective services to the families we support.
Maintaining Contract Capacity:
LifeTran Corporation has always prioritized capacity-building as a core aspect of our operations.
We recognize the importance of maintaining sufficient staffing and resources to meet service demands effectively. While we have successfully maintained and exceeded capacity in key regions, there are occasional challenges in rural areas where limited cases contribute to employee turnover. To mitigate this, we closely monitor waitlists and take immediate action to address any capacity shortfalls.

Currently, we have successfully tackled and eliminated almost all waitlists in the rural regions, and we are always diligently working to ensure comprehensive service accessibility across all regions we serve statewide. Our approach is proactive, ensuring that no community/region is underserved and that all families receive the support they need without unnecessary delays.
LifeTran Corporation is widely recognized as one of the most steadfast, dedicated, and dependable agencies among our peers. Our commitment to excellence, reliability, and unwavering service to the families and children we support sets us apart. We remain humble in our accomplishments yet driven in our mission to continually exceed expectations and reinforce our reputation as a trusted partner to DCS.
Our strategy includes continuous job postings, targeted recruitment efforts, and offering incentives to attract and retain staff in underserved regions. By doing so, we ensure that we remain fully equipped to meet and surpass the expectations set by DCS.
Overall, LifeTran Corporation remains steadfast in its commitment to delivering high-quality, reliable, and safe parenting time services. Our proactive approach to addressing challenges, coupled with our dedication to capacity-building, ensures that we continue to meet and exceed the expectations of the families and agencies we serve.

Question 4: Describe your agency's experience in partnering with Child Welfare entities and/or other related fields, including examples that required collaboration and coordination.
LifeTran, from inception as an entity, has always believed in the efficacy of collaborative engagements with other entities within the child welfare service provision circle to deliver desired outcomes for children and families.
We had joined forces with organizations such as juvenile services, group homes, churches and several charitable organizations to bring succor to families in need through their social investment programs and other set out platforms that equally had positive bearings in the unification of families as well as alignments of its value system.
However, and perhaps with nostalgia, partnering with ADCS since August 2017, to foster family cohesiveness has been a massive success, albeit exciting and positively challenging. We have, through this symbiotic relationship, utilized ADCS social and scientific approach in collaboration with its astute ADCS professionals, achieved tremendous results that have further scaled up, deepened, and widened the frontier of family reunification in almost all counties in Arizona that we serve, such that we could never have imagined possible prior to the collaboration.
LifeTran also understands that partnership is a critical strategy for driving innovation, knowledge and fostering growth in today’s fast-paced world, hence our consistency in utilizing its mechanisms to build momentum in our field of engagements. Our experience in partnering with
Child Welfare entities over the years has been tremendous.
In partnering with Child Welfare entities and/or other related fields, we have had multi-layer experiences in delivering services to families as well as work experiences with assigned human and social welfare professionals. For instance, through Supervised Visitations (parenting time) in partnership with ADCS, we utilized and align our well-trained staff to ensure they follow set-down

Office of Procurement & Contracts rules of engagements, and time-bound ADCS procedural roadmap to ensure and achieve positive outcomes, particularly knowing that we must overcome the following challenges and huddles to achieve the set goals, viz:
• cultural/language diversities
• environmental challenges
• personality and individual differences
• multiple causative factors etc.
Factoring the above amongst others, LifeTran pool of professional staff are culturally and socially diverse and equipped to fit into these buckets of diversities and provide intentional and channeled service solutions to the families we serve on one hand, whilst collaborating with staff of ADCS to adopt its Scope of Work, in the successful achievement of desired outcomes. Examples of ongoing LifeTran’s successful partnership with ADCS that require such collaboration and coordination are outlined in the several programs we currently provide for DCS namely, Clinically
Supervised Parenting Time, Family Connections, Nurturing Parenting Program and SENSE.
Through these programs, we have successfully.
✓ collaborated with Case Managers to carry out challenging cases with multiple families placed in different cities and locations.
✓ collaborated to ensure that families were served in the peak and downtime of Covid.
Working with Social Workers outside of DCS, Parole Officers, Court Clerks, Transporters, Placements, Schools, Therapists, Counsellors, and healthcare professionals to ensure that services were rendered to our clients without any hinderances.
✓ Efficiently collaborated with the assigned ADCS Specialist to provide visitation between parents and child(ren) that are out of state.
✓ Supervised numerous times, sibling only visitations through collaboration with the ADCS
Specialist and Placements and we take pride in making sure that the child(ren) do not feel left out. We have never declined any sibling visitation when assigned, rather we go above and beyond to ensure that the siblings can visit one another.

✓ We engage and participate in all ADCS Provider Collaboration meetings, roundtable discussions, and work groups with other agencies and contribute our quota when necessary.
✓ Because we have adequate rooms in our visitation centers in Tucson, Sierra Vista and
