Company Name: Enter the name of the company completing the form, i.e. your name.

Project Name: Enter the name of the project that is being used to support your qualifications.

Contracting Agency for Project Noted Above: Enter the name of the company, state agency, or Federal agency that issued a contract for this project.

Entity with whom you had a Contract: Enter the name of the entity that you contracted with.

This might be the Contracting Agency above if you were the prime contractor or another contractor if you were a subcontractor on the project in question.

Contract Number: Enter the contract number assigned to this project by the Contracting Agency.

Contract Amount: Enter the dollar amount of your contract on this project.

Start Date: Enter your start date on this project. If you were a subcontractor, this may not match the project's start date.

End Date: Enter your end date on this project. If you were a subcontractor, this may not match the project's end date. If work not complete, enter your scheduled end data and the percentage of your work complete.

Contracting Officer's Name: Enter the Contracting Officer's name for the Contracting Agency above.

Contracting Officer's Telephone Number: If known, enter the Contracting Officer's phone number.

Contracting Officer's Address: If known, enter the Contracting Officer's address.

Type of Construction (NAICS): Provide the NAICS code for the type of construction for the referenced project. If you do not know, choose 'unknown' from the drop down list.

Brief Description of Work: Provide a description of the work that you performed on this project.

Description of Quantities of Work: Provide a numerical list or description of quantities of work that you performed on this project.

Description of Project Complexity: Describe any difficulties that required more than basic skills to complete as well as any other items that you had to overcome to complete this project.

Remember the note on Form 245/245A that states:

Scope refers to technical requirements of work.

Complexity refers to difficulty of performing work due to many varied interrelated parts.

Similar in scope refers to the same type of work with comparable or greater quantities.

Your answers to 'Brief Description of Work,' 'Description of Quantities of Work,' and 'Description of Project Complexity' need to be sufficient that we can judge the work to be similar in scope and complexity to our solicitation.
