ADDENDUM TO CLAUSE 52.215-1
SPECIFIC INSTRUCTIONS TO OFFERORS
RFP 140P6218R0001
A. To be eligible for this Service-Disabled Veteran-Owned Small Business (SDVOSB) contract, the Offeror, when submitting its offer, must represent to the Contracting Officer that it is a SDVOSB concern under the NAICS code assigned to the procurement, which is NAICS 562910. If Offeror is not already registered and active in the System for Award Management (SAM), and/or have not completed the Representations and Certifications at https://www.sam.gov prior to submission of its offer to the Contracting Officer, the offer will be ineligible for consideration. The Offeror’s SAM registration and Representations and Certifications will be verified by the Contracting Officer during the Contracting Officer’s initial review of offers received. If it is found the Offeror is not registered and active in SAM, and/or the Offeror has not represented itself on its Representations and Certifications as a SDVOSB in the NAICS code of 562910, the Offeror will be notified that its offer is ineligible for consideration.
Note: As part of the Offeror’s proposal submission, in the Clauses and Provisions of the solicitation, under Provision 52.204-8 Annual Representations and Certifications, paragraph (b)(2), Offeror is to select either block (i) or (ii), as applicable, and submit that page with its proposal.
B. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of three (3) separate parts: Factor I – Technical Proposal, Factor II – Past Performance, and Factor III – Price Proposal. Proposals shall be submitted via email to the Contracting Officer, Kathryn Logsdon, at kathryn_logsdon@nps.gov. All documents submitted in Offeror’s proposal shall be provided in Adobe PDF Format. Response to this solicitation must be submitted so they are received by the due date and time specified below:

C. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, price competition is insufficient to support a determination of price reasonableness, additional price information or a detailed breakdown of pricing information to support a price reasonableness determination will be requested from the Offeror, at which time the Offeror shall promptly furnish.
D. SPECIFIC INSTRUCTIONS:
1. FACTOR 1 – TECHNICAL PROPOSAL. Offerors shall prepare and submit a technical proposal by the date/time stated above. The offeror’s technical proposal consists of three (3) subfactors:
(a) Plan for Execution, (b) Proposed Performance Schedule, and (c) Technical and Financial Capability. All subfactors are of equal importance.
(a) Subfactor 1: Plan for Execution. Offeror’s Plan for Execution shall not exceed TEN (10) pages in length. Pages shall be numbered and shall have a header or footer that contains the offeror’s name and the solicitation number applicable to this RFP.
Written plan includes, at a minimum, a narrative detailing Offeror’s management approach that demonstrates a reasonable methodology to using sound business practices to accomplish and fulfill the requirements of the contract, to include: risk management, logistics, mobilization, safety, quality control and resolution of potential issues and challenges. Offerors shall also include proposed methods to ensure safety of park employees and park visitors. The written plan shall also include, at a minimum, a narrative detailing Offeror’s technical approach/capabilities that provides evidence of Offeror’s thorough understanding of the work to be performed, required permits, applicable codes and disciplines, and environmental compliance to fulfill the requirements of the contract.
(b) Subfactor 2: Proposed Performance Schedule. Submit a proposed performance schedule assuming a Notice to Proceed is issued on January 26, 2018. Schedule shall be in Bar (Gantt) Chart format. Schedule shall capture major tasks/milestones of the project requirements, execution timelines, dependencies between tasks/milestones, and critical path items. The schedule must demonstrate coordination of the work activities, phasing, and potential weather considerations.
(c) Subfactor 3: Technical and Financial Capability. Submit one (1) completed Technical and Financial Capability Statement. Use the Government-provided form included in the solicitation as Attachment 5; do not alter the form. Additional pages may be needed to provide all information requested by the form. Additional pages shall be numbered and clearly marked that the pages are a supplement to the Technical and Financial Capability Statement. The total number of additional pages supplementing the Technical and Financial Capability Statement shall not exceed TEN (10) pages in length. Mark each page with offeror’s name and the solicitation number applicable to this RFP.
The offeror’s responses furnished in Attachment 5 - Technical and Financial Capability Statement and its supplemental pages must be complete, and evidence the offeror possesses or has the ability to provide the (1) technical knowledge, (2) technical capability, (3) licenses, (4) relevant experience, (5) resources (to include financial), and (6) business, managerial, organizational, and administrative acumen to successfully provide and perform all requirements of this contract.
Licenses – Offerors are to submit copies of the appropriate licenses with their proposal.
Firms involved in the business of abating regulated asbestos containing material (RACM) must have a contractor’s license from the Indiana Department of Environmental Management (IDEM) and be an Indiana licensed Asbestos Abatement contractor.
Individuals involved with abating RACM, inspecting for RACM, or conducting management planning activities must have an individual license from IDEM and be an Indiana licensed Asbestos Abatement Worker, Supervisor, Inspector or Management Planner.
Relevant experience of key personnel:
(1) Project Manager: Must be supported by a resume of the capable individual who will be responsible for the organization and administration of the project. Responsibilities of a Project Manager under this contract would include, but are not limited to:
(i) understanding and managing of the contract,
(ii) knowledge of environmental and OSHA regulations and compliances relating to the work to be performed, materials to be acquired, and the tools, equipment and practices to be used under the contract (not needed by Project Manager if the construction site supervisor on the project possesses this knowledge),
(iii) communications and resolution of contract matters and issues,
(iv) ensuring effective and strategic coordination of contract activities, logistics, manpower, and materials, and
(v) documents control and submission (e.g., project and material submittals, certified payrolls, insurance, bonds, schedule of values, construction schedules, safety/accident prevention plan, test results, inspections, progress reports, subcontractor documentation, O&M manuals, warranties, invoices, etc..).
(2) Construction Site Supervisor: Must be supported by a resume of the capable individual(s) who will be supervising and overseeing the work at the contract work site. Responsibilities of a Construction Site Supervisor under this contract could include, but are not limited to:
(i) knowledge and familiarity of the materials to be used, the methods, means, and regulations for asbestos abatement, construction disciplines, and the resource skills needed for successful completion of the asbestos abatement and construction work of this contract.
(ii) demonstrate leadership and provide on-site coordination for all phases of the work to be performed, including hiring/scheduling of crew, coordinating subcontractors, material and equipment, ensuring that specifications are being strictly followed, and work is proceeding on schedule and within budget.
(iii) responsible for scheduling, inspections, quality control, job site safety, acquiring necessary permits, and compliance to the government’s security regulations and procedures.
(iv) knowledge of environmental and OSHA regulations and compliances relating to the work to be performed,.
(v) maintain open and constructive communications, and attendance at project progress meetings with government project personnel,
(vi) completion and submission of project reports such as daily logs, certified payrolls, material submittals, records of meetings, etc.
(3) Environmental Manager: Must be supported by a resume of the capable individual who has a minimum of 3 years construction experience on projects of similar size and scope; with environmental procedures similar to those of this project, familiarity with Environmental Management Systems (EMSs) such as ISO 14001; and must be familiar with environmental regulations applicable to construction operations.
The Environmental Manager will oversee the Contractor’s conformance to environmental goals for the project and implementing procedures for environmental protection, to include but not limited to:
implementation of the Waste Management Plan, Implementation of the Indoor Air Quality Management Plan, training for contractor personnel, and monitoring and documentation of environmental procedures.
(4) Historic Preservation Treatment Specialist: Must be supported by a resume of the capable individual(s) who has the experience through past performance of not less than three (3) years in historic building preservation, and has a degree or certification in the study of historic preservation from an institution of higher learning such as those listed at website www.preservationdirectory.com. The degree or certification must be relevant to historic building preservation.
(5) Professional Engineer: Must be supported by a resume of a professional engineer who is legally qualified to practice in jurisdiction where Project is located and who is experienced in providing engineering services of the kind indicated (including Structural Tests and Special Inspections (STSI).
Engineering services are defined as those performed for installation of the system, assembly, or products that are similar to those indicated for this Project in material, design, and extent.
(6) Installer: Must be supported by the capabilities statement of a firm, or resume by a capable individual experienced in installing, erecting, or assembling work similar in material, design, and extent to that indicated for this Project, whose work has resulted in construction with a record of successful in-service performance.

2. FACTOR II – PAST PERFORMANCE. Offeror shall submit past performance information requested by Attachment 6. The offeror must support a relevant work history of performing on similar projects of at least the size, scope, and complexity of a contract resulting from this solicitation, and/or non-relevant work history where experience and/or knowledge was gained that offeror believes would be of benefit to the government in overall performance of the work described in the solicitation. The past performance claimed by Offeror must be supported by at least three (3) of the Offeror’s references when contacted by the contracting officer or technical evaluator. The Government will utilize the information provided, along with information the Government may collect from other sources (i.e., other Government offices and reporting systems, commercial sources, etc.), in rating the offeror’s past performance.
3. FACTOR III – PRICE PROPOSAL - The solicitation constitutes the model contract.
Offerors are to submit the documents and information described in Subfactors 1, 2, and 3 below.
Offerors not completing and/or submitting the documents/information for these subfactors may be deemed ineligible for award. Alternate Line Item Proposals will not be considered.
(a) Subfactor 1: Attachment 4 – Offeror’s Proposed Price Schedule
(1) Page 1 - Complete all information requested for Line Item 10 – Asbestos and Construction. Complete the Name of Firm, Signature, Title and Date at bottom of page.
(2) Page 2 - Complete all information requested for Itemization of Miscellaneous Expenses. Complete the Name of Firm, Signature, Title and Date at bottom of page.
(b) Subfactor 2: SF1442 - Solicitation, Offer, and Award Form
(1) Complete blocks 14, 15, and 16 with Offeror’s information. Also include Offeror’s DUNS number.
(2) Complete block 17
• Complete the number of calendar days the Offeror has given the Government for the Government’s acceptance of Offeror’s proposal, AND
• Fill in the Amount with Offeror’s Total Proposal Price (as priced in Attachment 4).
(3) Complete block(s) 19 acknowledging Offeror’s receipt of all amendments to the RFP.
(4) Complete, date and sign blocks 20a, 20b, and 20c. In doing so, the Offeror accedes to the contract terms and conditions as written in the solicitation, to include attachments.
(c) Subfactor 3: Proposal Guarantee
(1) Offeror will furnish a proposal guarantee in the proper form and in the amount as required and described in Clause 52.228-1.
(2) The Offeror’s proposal guarantee must be furnished to and received by the Contracting Officer no later than the solicitation closing date and time. The proposal guarantee shall be the signed original and sent to:

E. General Information
1. Offerors failing to provide any of the above information may result in the entire offer being excluded from further competition.
2. Late Proposals. Late proposals will be processed in accordance with FAR 52.215-1(c)(3), Submission, modification, revision, and withdrawal of proposals.
3. The Contracting Officer will acknowledge receipt of proposals received. Acknowledgement will be by return e-mail from the Contracting Officer indicating the date and time the offeror’s proposal entered the Contracting Officer’s e-mail inbox, along with identifying the number of attachments received. Should offeror not receive a return acknowledgment within 24 hours from the time the offeror’s proposal was e-mailed, or should the number of attachments acknowledged by the Contracting Officer be in error, contact the Contracting Officer by phone at (217) 391-3225, or by e-mail at kathryn_logsdon@nps.gov, immediately after the 24-hour acknowledgement period.
4. In accordance with FAR Subpart 4.8, "Government Contract Files," the government will retain one copy of all unsuccessful proposals.
