About this file
This document is a Past Performance Questionnaire (PPQ) template for a solicitation by the United States Fish and Wildlife Service (FWS) for security guard services. The questionnaire is intended to be completed by government agencies that have previously contracted with the offeror, in order to evaluate the offeror's past performance.
The questionnaire covers key areas such as the offeror's understanding of technical requirements, resource management, workforce quality and retention, innovation, problem resolution, quality of deliverables, schedule management, compliance, and overall performance. Agencies are asked to provide adjectival ratings and comments on the offeror's performance in these areas. The questionnaire also includes questions about whether the offeror received any cure or show cause notices, if they are rated in CPARS, and if the agency would recommend the offeror for award again. The completed questionnaires are to be returned to the offeror for submission with their proposal, which is due on July 18, 2024.

June 18, 2024
FOR OFFICIAL USE ONLY
PAST PERFORMANCE EVALUATION QUESTIONNARIE FORM
The United State Fish and Wildlife Service (FWS) is solicitation proposals for the security guard services.
One of the considerations in proposal evaluations is the verification of the offeror’s past and present performance on contracts, which reflect the offeror’s ability to perform on the proposed effort. The Government depends on information received from Agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror’s performance on those contract(s).
The areas of interest for the Government are summarized in the questionnaire below. Questionnaires should be completed and returned to the offeror for submission with their quotes. Quotes are due July 18, 2024, so we kindly ask for your respond by July 11, 2024 so it can be included in the quote submission.
Please complete all sections of the attached questionnaire. Include your name and title, organizational address, email address, telephone number. Use the rating scale and definitions found in the questionnaire to rate each performance element. Please sign the questionnaire.
The Government encourages comments. The last page may be used if additional space is needed for comments. Clear handwritten responses are sufficient.
Thank you for your time and participation.
Sincerely, Joni Dutcher Contract Specialist
