About this file

This document is a Subcontractor/Supplier Utilization Commitment form (CBI Form 3) for the City of Charlotte's Structural Engineering and Parking Deck Inspection Services project (Contract #269.2026.135). The form is designed to capture information about Minority, Women, and Small Business Enterprise (M/W/SBE) subcontractors and suppliers that a proposer intends to use across all tiers of the contract. The form must be submitted at the time of Proposal Opening, and failure to properly complete it can result in proposal rejection.

The document outlines specific requirements for listing M/W/SBE and non-M/W/SBE subcontractors and suppliers, including the need for vendors to be currently certified with the City. Proposers must maintain the level of M/W/SBE participation throughout the contract duration, submit Letters of Intent within three business days if requested, and ensure all subcontractors and suppliers are registered in the City's vendor database. The form includes provisions for adding subcontractors after initial submission and mandates compliance with the City's CBI Administrative Procedures Manual, with potential proposal rejection for improper documentation.
