About this file
This is a Vendor Responsibility Questionnaire (VRQ) document required by the State of Georgia for formal solicitations exceeding $100,000 and/or involving public health, infrastructure, or operational service delivery. The VRQ is designed to proactively mitigate procurement risk and ensure vendor integrity in compliance with applicable Georgia procurement laws, including O.C.G.A. 50-5-50, 50-5-82, 50-5-84, 84.1, 84.2, and 13-10-91. The questionnaire requires vendors to complete five sections comprehensively: General Vendor Identification, Legal & Regulatory Compliance, Financial and Operational Stability, Tax Compliance, and Certification. All fields are mandatory unless marked as not applicable, and vendors must provide detailed written explanations and supporting documentation for any affirmative responses in the Legal & Regulatory Compliance section. The final page must be notarized with a valid commission expiration date included.
The questionnaire addresses critical compliance areas including debarment history, environmental law violations, OSHA citations, procurement fraud convictions, scrutinized or foreign company status under Georgia law, contract completion failures, ongoing investigations, financial capacity, insurance maintenance, bond procurement ability, and tax compliance status. Vendors must certify under oath that all information provided is accurate and complete, that they are authorized to bind the firm contractually, and understand that failure to disclose relevant information or falsification may result in immediate disqualification or contract termination. The VRQ also requires E-Verify compliance affirmation for public works projects and parent company disclosures for subsidiaries or affiliates.
