About this file

This Statement of Work outlines requirements for constructing emergency exit walkways at the Yakama Service Unit Health Clinic in Toppenish, WA, operated by Indian Health Service. The project addresses safety violations where dental, medical waiting, and laboratory emergency exits terminate into a yard without proper egress routes.

The contractor must construct concrete walkways meeting NFPA 101 and IBC 10 standards, including 4-inch gravel base, Grade 60 steel rebar reinforcement, and specific concrete mix requirements per Washington State DOT specifications. Key deliverables include shop drawings, safety plans, materials documentation, and post-construction testing reports. The work requires a Washington state licensed general contractor, OSHA-certified site supervisor, and compliance with state building codes, OSHA regulations, and the Architectural Barriers Act. Quality assurance includes concrete testing for temperature, slump, air content, and comprehensive strength, with specific requirements for materials, installation, and finishing detailed in WSDOT specifications.

Yakama Service Unit, Main Clinic: 401 Buster RD, Toppenish, WA 98948

A sitemap is included as Attachment A.

B. PROBLEM STATEMENTS

The Yakama Health Clinic Pharmacy, Dental, Medical waiting and Laboratory emergency exits are identified as fire exits by the Yakama SU fire plan. There’s a lack of designated and clear pathways which poses a safety risk during emergencies.

C. BACKGROUND

The Yakama Indian Health Center is a single-story structure located in the city of Toppenish, Washington on the Yakama Reservation.
