About this file

This document is an Arizona Department of Transportation (ADOT) attachment titled "Forced Labor of Ethnic Uyghurs Ban," a compliance certification form required for state contracts. The document pertains to Arizona Revised Statutes (A.R.S.) § 35-394, which prohibits public entities in Arizona from entering or renewing contracts with companies that use forced labor of ethnic Uyghurs in the People's Republic of China. The form applies to companies with ten or more full-time employees and requires contractors to certify their non-involvement with forced labor practices.

The specific document is completed by Superstition Collision Center, located at 1315 N Marlin Ct. in Gilbert, Arizona, for Solicitation No. BPM006654. The contractor has affirmed that they do not use, and will not use during the contract term, any forced labor of ethnic Uyghurs, nor will they use goods or services produced by such labor, or engage with contractors, subcontractors, or suppliers involved in such practices. The form provides an exemption for sole proprietorships, companies with fewer than ten employees, and non-profit organizations, though in this case, the contractor appears to be a for-profit business with more than ten employees.
