About this file

This is an Anti-Human Trafficking Affidavit form required by Broward County, Florida for vendors seeking to contract with the Port Everglades Department and various other Broward County agencies for repair or replacement of windows and glass doors. The affidavit must be completed and submitted with the vendor's proposal, or within three business days of the County's request, and upon any award, renewal, or extension of contract. Vendors who fail to comply with the submission requirement within the stated timeframe may be deemed nonresponsive. The form, dated September 9, 2024, requires the vendor to certify under penalties of perjury that they do not use coercion for labor or services as defined in Section 787.06, Florida Statutes, and requests the vendor's name, authorized signature, printed name, title, and date.

This affidavit is a compliance document rather than a pricing or contractual terms document. It serves as a mandatory certification requirement for all vendors participating in Broward County solicitations and does not address compensation, set-asides, funding sources, or contract duration. Submission is required electronically through the BPRO platform by the solicitation closing date and time, and vendors are encouraged to submit documentation in advance to allow for timely and comprehensive county review.
