About this file

This document is an Affirmation and Disclosure Form from EnviroTech Services, LLC, addressing contractual obligations with the Ohio Department of Transportation (ODOT). The form specifically focuses on compliance with Executive Order 2022-02D, which prohibits the purchase of services from Russian companies or institutions, and reinforces previous restrictions on offshore services. The contractor affirms that all services will be performed within the United States and that state data will not be sent or accessed outside the country without a signed waiver.

The document does not specify the exact goods or services being contracted, but it requires the contractor to disclose the location of its business, service performance, and data storage. EnviroTech Services, LLC is located in Greeley, Colorado, and has committed to performing services as requested by Ohio DOT at a location to be determined. The form emphasizes the contractor's obligation to immediately notify the state of any changes in service location and acknowledges that ODOT retains the right to terminate the contract if services are moved outside the United States without proper authorization.
