This document is an Arizona Department of Transportation (ADOT) Confidentiality and Trade Secrets Attachment form, part of a solicitation response process for a state contract. The document involves KWC International as the proposing company, located in Phoenix, Arizona, and is associated with Solicitation No. BPM006654. The form is designed to help the state determine whether materials submitted in a solicitation response contain proprietary or trade secret information that should be protected from public disclosure.

The form specifically requires the proposing company to indicate whether their response contains trade secret information by selecting one of two options. In this case, KWC International has marked that their response does not contain proprietary or trade secret information, meaning their entire response will become a public record in accordance with Arizona Administrative Code (A.A.C.) R2-7-C317. By completing this form, the company agrees to indemnify and hold the state harmless from any claims related to the withholding of information, and commits to cooperating with any potential challenge to their designation of trade secrets or confidential information.
