About this file
This is a subcontractor list submission form for Walworth County, Wisconsin, related to a Request for Qualifications (RFQ) #26-032 for Emergency Mitigation Services. The form requires bidders to submit a comprehensive list of proposed subcontractors as part of their bid submission, including the class of work each subcontractor will perform and the estimated value of their work. Per Wisconsin Statutes Section 66.0901(7), each subcontractor must submit their bid in writing to the general contractor at least 48 hours prior to the bid closing deadline. The document serves as a standardized template with designated spaces for subcontractor names, their respective classes of work, and estimated contract values across multiple line items.
The form emphasizes that while subcontractor listings are required as part of the bid package, omission of a subcontractor from the submission does not invalidate a bid. Importantly, no subcontract—whether listed on this form or proposed later—may be executed without prior written consent from Walworth County. The document contains largely blank fields, indicating this is a template form awaiting completion by prospective bidders rather than a completed submission with specific subcontractor information.

LIST OF SUBCONTRACTORS
Section 66.0901(7) of the Wisconsin Statutes provides that a bidder, as a part of his bid, shall submit a list of the subcontractors he proposes to contract with and the class of work to be performed by each, provided that to qualify for such listing, each subcontractor must first submit his bid in writing to the general contractor at least 48 hours prior to the time of bid closing. It further provides that a bid of a bidder shall not be invalid if any subcontractor, and the class of work to be performed by such subcontractor, has been omitted from a bid.
No subcontract, whether listed herein or later proposed, may be entered into without the written consent of the Walworth County.
