About this file
This file contains prequalification submittal and oral interview evaluation forms for the State of Colorado Office of the State Architect State Buildings Program related to the Statewide Real Property Development Plan (RFQ 082613). The evaluation forms are designed to assess the qualifications of firms submitting proposals for architectural and related professional services. The prequalification submittal evaluation form assesses firms across six primary categories: Project Team qualifications and experience (weighted at 19 points total), Firm/Team Capabilities including management structure and energy modeling capability (15 points), Prior Experience with similar projects and commissioning (13 points), Project Approach including value engineering and schedule management (19 points), Equity, Diversity and Inclusion initiatives (0 points as weighted), and Work Location proximity and familiarity with project areas (10 points). Evaluators rate each criterion on a scale of 0 to 5, with scores calculated by multiplying the assigned weight by the numerical rating for each criterion.
The oral interview evaluation form follows a similar assessment methodology with four primary criteria: Project Team (weighted at 3 points), Team Capabilities (3 points), Prior Experience (4 points), and Project Approach (4 points), for a total of 14 available points. Both evaluation forms require evaluators to verify that minimum requirements have been met, including submission of a letter from a surety, and to confirm that the Acknowledgment and Attestation have been included in the submittal. The forms provide flexibility for agencies and institutions to add project-specific criteria within each category to better assess submitters' overall qualifications for the particular project under consideration.
