About this file

This document is an Affirmation and Disclosure Form (Revision 3/22) designed to ensure compliance with Executive Orders regarding offshore services and data location. The form requires a contractor to disclose the principal business location, service performance locations, and data storage locations for both the primary contractor and any subcontractors. The purpose is to prevent performance of services, data storage, or purchasing from Russian institutions or companies.

The form mandates that the contractor affirm and agree to immediately notify the State of any changes in service or data location before, during, or after contract execution. The State retains the right to terminate the contract if services are performed outside the United States, unless a signed waiver is obtained. The document is intended to be incorporated as part of any contract between the contractor and the State, with the contractor's authorized representative required to sign and acknowledge the terms, demonstrating understanding and compliance with the specified location and disclosure requirements.
