About this file
This document is an Affirmation and Disclosure Form, revised in March 2022, designed to ensure compliance with Executive Orders regarding offshore services and data location. The form requires contractors to disclose the principal business location, service performance locations, and data storage sites for both the primary contractor and any subcontractors. It specifically aims to prevent contractors from performing services offshore, storing state data outside the United States, or purchasing from Russian institutions or companies.
The form mandates ongoing transparency, requiring contractors to immediately notify the state of any changes in service or data location before, during, and after contract execution. Failure to provide complete location information may result in contract non-award. The state retains the right to immediately terminate the contract if services are performed outside the United States without a signed waiver. The document serves as a critical compliance mechanism for state contracts, ensuring geographical and institutional accountability in service delivery and data management.
