Imaging of filed case records.

The Florida Department of Health awarded a Master Agreement (SK246) on December 10, 2019, for imaging of filed case records. The contract establishes a framework agreement with a completion date of December 10, 2024, and carries a contract amount of $1,000.00. The master agreement structure allows for the issuance of blanket purchase orders against the established terms and conditions.

The primary line item under this agreement is a Blanket Purchase Order covering the period from July 1, 2026, through June 30, 2027, for on-site shredding services. Services are to be performed once monthly at two locations: the Haines City Dental Office and the Lakeland Dental Office. The minimum service charge is $22.35 for one 32-gallon container at the Haines City location and $37.05 for one 96-gallon container at the Lakeland location. The line item reflects a quantity of 1,000 units at a unit price of $1.00 each, consistent with the blanket purchase order structure for recurring monthly services.
