The National Institute of Neurological Disorders and Stroke (NINDS) awarded a $6,522,833 Project Grant to The Leland Stanford Junior University to conduct a mechanistic randomized clinical trial investigating the central and peripheral nervous system mechanisms of pain relief induced by peripheral nerve stimulation (PNS) for the treatment of refractory chronic neuropathic pain. The 3-year study, funded under the NIH HEAL Initiative's CFDA 93.279 Drug Abuse and Addiction Research Programs, aims to characterize biopsychosocial predictors of PNS treatment response. The project will enroll 134 patients with lower extremity chronic neuropathic pain due to peripheral nerve injury, randomizing them to conventional medical management or combined PNS and conventional treatment. Key activities include quantitative sensory testing, PET/MRI and PET/CT imaging with a novel sigma-1 receptor radiotracer, and remote outcome assessments over 1 year. The University of Kansas Medical Center Research Institute (KUMCRI) and University of Wisconsin-Madison are sub-awardees providing specialized expertise in study procedures and PET/MRI image analysis, respectively.
