About this award

The National Institutes of Health Office of the Director awarded the University of Southern California $1.7 million on September 15, 2026, under the Trans-NIH Research Support program (CFDA 93.310) to establish GLIMMER, a real-world surveillance platform investigating glucagon-like peptide-1 receptor agonist use and its relationship to cognitive aging and Alzheimer's disease and related dementias risk.

GLIMMER embeds three new data streams within the Understanding America Study, a nationally representative probability-based internet panel of approximately 14,000 U.S. adults with 12+ years of cognitive, behavioral, and cardiometabolic data. Monthly GLP surveillance across all participants captures near-real-time treatment dynamics; annual core surveys of identified users and non-users measure slower-changing domains; and deep phenotyping with body composition assessment, continuous glucose monitoring, and dietary ecological momentary assessment in consenting respondents generates detailed biomarkers. Together these streams capture new GLP initiators with pre-treatment baselines, track full treatment trajectories, and produce sample sizes, protocol evidence, and preliminary data for future research. The resulting publicly available resource will provide the first nationally representative, real-world platform linking lifecourse GLP use to cognitive aging and ADRD risk, addressing a gap in understanding whether GLP therapies prevent, delay, or accelerate cognitive decline in real-world populations outside controlled trials.

Work is performed in Los Angeles, California. The period of performance runs through August 31, 2027.

Description

Abstract
Glucagon-like peptide-1 receptor agonists and related therapies (glps) are now used by more than 37 million U.S. Adults, yet whether they prevent, delay, or accelerate cognitive decline and dementia in real-world populations remains unknown. Trials are short-term and highly selected; medical records miss the growing share of users accessing glps outside traditional clinical channels; and existing surveys lack timely GLP surveillance and objective phenotyping. Glimmer (GLP influence on metabolism and mind examined in real-world, real-time) is a nationally representative, high-frequency GLP surveillance platform embedded in the understanding america study (UAS), a probability-based internet panel of ~14,000 U.S. Adults with 12+ years of cognitive, behavioral, and cardiometabolic data. Glimmer adds three new data streams: monthly GLP surveillance across all UAS participants to capture treatment dynamics in near real time, annual core surveys of identified users and non-users to measure slower-changing domains, and deep phenotyping with body composition assessment, continuous glucose monitoring, and dietary ecological momentary assessment in consenting respondents. Together, these capture new initiators with pre-treatment baselines, track full treatment trajectories, and generate the sample size, protocol evidence, and preliminary data needed for future research. The resulting publicly available resource will provide the first nationally representative, real-world platform linking lifecourse GLP use to cognitive aging and adrd risk.
