This $472,448 federal Project Grant award from the U.S. Department of Housing and Urban Development (HUD) under the Continuum of Care (CoC) Program (CFDA 14.267) will support Transition Projects, Inc., a nonprofit organization in Portland, Oregon, in delivering a range of services to address homelessness in the local community.
The funding will allow Transition Projects to provide critical housing-related activities, including rapid rehousing, rental assistance, case management, and supportive services to quickly house homeless individuals and families while minimizing trauma and promoting self-sufficiency. The organization will leverage this CoC Program grant, along with its other federal funding sources such as Supportive Services for Veteran Families (SSVF) grants, to deliver comprehensive, integrated services focused on transitioning homeless populations into permanent housing and maintaining long-term stability. The grant period runs from March 11, 2025, through the end of that calendar year.
