LPCA will manage six small sub awards to 6 FQHCs (Federally Qualified Health Centers). The six clinics that will receive funding from LPCA will be responsible for activities and deliverables as stated in each centers signed letter of commitment and the following: Each clinic will designate a point of contact with the clinic with the authority to work collaboratively to make program related decisions to support increasing colorectal cancer screening; Provide opportunities and staff time for clinical staff to participate in continuing education, professional development, and quality improvement activities; Assist in identifying training needs and opportunities for their staff; Assign a clinic champion to participate in grant activities: baseline screening assessment, clinic assessment to determine appropriate strategies for increasing screening, implementation of screening policies, goals, and targets, implementation of evidence-based interventions, and ongoing quality improvement. Provide the appropriate staff person to participate in the Louisiana Colorectal Cancer Roundtable, if applicable; To work with practice facilitators who will provide technical assistance and quality improvement tools related to cancer screenings; and, Allow practice facilitator to attend monthly Ql meetings.
