About this award
The Department of Housing and Urban Development's Assistant Secretary for Community Planning and Development awarded $209,045 to the New Hampshire Department of Health & Human Services on October 1, 2025, under the Continuum of Care Program (CFDA 14.267) to support youth homelessness prevention and intervention services through the Youth Homelessness Demonstration Program (YHDP).
The award funds eligible costs for establishing and operating projects across five program components: permanent supportive housing for persons with disabilities, rapid rehousing, transitional housing, supportive services only, and homeless management information systems (HMIS). The YHDP specifically serves youth-defined as households where no person is over age 24-and aims to support development and implementation of coordinated community approaches to preventing and ending youth homelessness while mobilizing communities nationwide toward the same objective.
Performance occurs in Concord, New Hampshire, with a period of performance from October 1, 2025, through September 30, 2026. The New Hampshire Department of Health & Human Services serves as the recipient and administers the award, distributing funding and services statewide to prevent and end youth homelessness through a coordinated continuum-of-care delivery model.
