This Project Grant award of $1,578,750.00 from the U.S. Department of Housing and Urban Development (HUD) Continuum of Care (CoC) Program (CFDA 14.267) supports the City of Baltimore's efforts to address homelessness. The funding will cover a one-year period from October 1, 2024 to September 30, 2025.
The grant provides resources for the City of Baltimore to deliver a range of homeless services, including permanent housing, transitional housing, supportive services, homeless management information systems, and homelessness prevention. As a sub-recipient, Health Care for the Homeless Inc. will utilize $XX,XXX.XX of the funding to operate 55 units of permanent supportive housing for chronically homeless individuals, providing integrated health care, case management, peer support, and other wraparound services to promote housing stability and self-sufficiency. The comprehensive, community-based approach aims to quickly rehouse homeless individuals and families while minimizing the trauma of homelessness.
