However, no patient will be turned away. Mobile integrated healthcare services will be provided in ST. Charles county and will include primary care, behavioral health services, SUD/OUD services, specifically, field initiation of buprenorphine. The MIH network will also provide resources and services to address health inequities and social determinants of health. Partners include preferred family healthcare, compass health, and the ST. Charles fire department, among others. Whole person care will be delivered through the evidence-based mobile integrated healthcare model. The goals of the project are to improve referrals to appropriate treatment and recovery supports for 600 high-risk individuals over the four-year project period, increase local workforce capacity to engage, treat and support recovery of individuals with SUD/OUD and/or behavioral health needs, develop a replicable and sustainable MIH payment model supported locally and by medicaid and other payers, and meet all grant deliverable on time and within budget. The objectives are to identify and enroll up to 600 individuals in MIH, provide SUD/OUD training and education to 810 individuals annually or 3,240 over the four-year period, secure support of payers for MIH service delivery and meet all grant deliverables.
