This Project Grant award of $1,618,354 from the Substance Abuse and Mental Health Services Administration (SAMHSA) under the Substance Abuse and Mental Health Services Projects of Regional and National Significance (CFDA 93.243) program will fund the implementation and dissemination of the Collaborative Care Model (CoCM) to improve care for individuals diagnosed with mental illness and substance use disorders in 12 medically underserved counties in Pennsylvania. The Pennsylvania Department of Human Services (PA DHS) will partner with UPMC, a large integrated delivery and finance system, to establish the CoCM and increase collaboration between medical and behavioral health providers. This model is designed to improve patient outcomes, reduce costs, and empower patients and their families. The project will serve residents of the targeted Pennsylvania counties, which make up approximately 16% of the state's population, of which an estimated 6.5% have co-occurring mental health and substance use disorders. The award period runs from September 30, 2024 to September 29, 2029.
