The New Jersey Department of Human Services (NJ DHS) was awarded a $900,000 Project Grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) under the Substance Abuse and Mental Health Services Projects of Regional and National Significance (CFDA 93.243) program. The grant will be used to implement the Collaborative Care Model (COCM) in two primary care and one pediatric care clinics located in Newark and Elizabeth, two Medically Underserved Areas in New Jersey. The project will serve 1,100 unduplicated individuals with serious mental illness, serious emotional disturbance, and co-occurring physical health and/or substance use disorders who present at the participating clinics. The initiative will utilize teams of psychiatric consultants, behavioral health care managers, nurse care managers, and support staff to coordinate behavioral health services with primary care. Key goals include increasing identification and treatment of mental health and substance use disorders, improving patient attendance and retention in care, monitoring metabolic parameters for those receiving antipsychotics, using measurement-based care to enhance depression treatment, and screening children for depression and anxiety. The project period is from September 30, 2024 to September 29, 2029.
