The service area spans a ten-county region in southwestern Oklahoma (Caddo, Comanche, Cotton, Grady, Harmon, Jackson, Jefferson, Kiowa, Stephens, and Tillman counties) within the Indian Health Service Lawton Service Unit, serving AI/AN individuals of all ages. Place of performance is Anadarko, Oklahoma. The period of performance runs from September 30, 2026, to September 29, 2029.
