This $750,000 project grant from the Department of Justice Office of Justice Programs Bureau of Justice Assistance will support Westcare Tennessee, Inc.'s Breaking the Cycle reentry initiative. The initiative will provide comprehensive substance use disorder assessments, community-based SUD treatment, intensive case management, and wraparound services to 75 adult men and women diagnosed with SUD or co-occurring mental health disorders returning to Cocke and Jefferson Counties, Tennessee from incarceration. Westcare Tennessee will partner with the Cocke County Jail to deliver pre-release case management, continue care coordination post-release, and connect participants to a range of recovery support services including primary medical care, HIV/hepatitis testing, transportation, employment assistance, housing, life skills training, and parent education. The jail will identify and refer eligible individuals to Westcare for assessments using the Ohio Risk Assessment System and American Society of Addiction Medicine criteria to develop individualized treatment plans. Services will be tailored to participant needs and continue until goals are achieved. This project supports the Second Chance Act Reentry Initiative's goal of reducing recidivism among those returning from prisons and jails.
