PNQCRI CONTINUES ITS INITIAL FORMATION AND RECOGNIZES THAT ACHIEVING HEALTH EQUITY REQUIRES ACTION, LEADERSHIP, INCLUSION, CROSS-SECTORAL COLLABORATION, AND SHARED RESPONSIBILITY WITHIN HOSPITAL NETWORKS, PROVIDERS, COMMUNITY ADVISORS, AND HEALTHCARE TEAMS.
