The Centers for Medicare and Medicaid Services (CMS) awarded a $1,655,999 Cooperative Agreement under CFDA 93.694 "Section 206 Consolidated Appropriations Act, 2024: State Planning Grants to Promote Continuity of Care for Medicaid & CHIP Beneficiaries" to the State of Alaska Department of Health. The funding will be used to enhance continuity of care for justice-involved youth transitioning out of carceral settings in Alaska. Key activities include: Developing an integrated communication and health management system between the Alaska Department of Health, Department of Corrections, and Department of Family and Community Services to ensure timely identification, tracking, and care coordination for justice-involved youth. Providing the necessary technological infrastructure to enable carceral agencies to deliver telehealth services to justice-involved youth in the 30 days prior to their release, ensuring access to pre-release health interventions. Preparing carceral agencies to enroll as Medicaid providers and establish the capability to bill within the Medicaid Management Information System (MMIS). The project aims to reduce barriers to care, promote continuity of care upon reentry, and ensure access to Medicaid and other health services for this vulnerable population, ultimately contributing to reduced recidivism rates and improved overall well-being.
