About this program

The Centers for Medicare and Medicaid Services (CMS) under the Department of Health and Human Services administers the Medicare Prescription Drug Coverage program (CFDA 93.770) to subsidize prescription drug benefits for Medicare beneficiaries through voluntary enrollment in prescription drug plans, with additional subsidies targeted to lower-income participants.

This program does not operate as a traditional grant-making vehicle for project-based or capacity-building funding. Instead, it functions as an ongoing subsidy and contracting mechanism: CMS makes periodic payments directly to prescription drug plan sponsors—risk-bearing entities organized and licensed under state law—to cover the cost of prescription drug benefits delivered to enrolled beneficiaries. Plan sponsors may offer coverage as a standalone benefit or in conjunction with Medicare Advantage plans and must meet specific organizational and financial requirements under 42 CFR 423.504 and 423.505. Contracts with sponsors renew annually, and the program operates on an indefinite authorization.

Eligible applicants are entities organized and licensed under state law as risk-bearing entities eligible to offer health insurance in each state in which they seek to operate. Rejected applications may proceed through a reconsideration and appeal process. Beneficiaries enrolled in plans have similar recourse for adverse coverage determinations, and CMS conducts periodic audits of plan cost reporting and financial records—covering at least one-third of participating plans annually—with additional audit authority delegated to the HHS Office of Inspector General and the Comptroller General.

Because this program channels federal payments to plan sponsors rather than awarding grants to develop new projects or services, the traditional portfolio metrics of award count, dollar concentration by funder, and recent opportunity postings do not apply. The source material does not document individual grant awards, award amounts, recent funding opportunities with application deadlines, or historical trends in award frequency. Organizations interested in participating should contact CMS directly regarding plan sponsorship eligibility, contract renewal timelines, and current enrollment periods under the established regulatory framework in 42 CFR 423.
