About this program

The Centers for Medicare and Medicaid Services (CMS) awarded its most recent grant under CFDA 93.634 on November 2, 2020; no grant opportunities are currently open under this program, and the last posted solicitation closed on August 17, 2020.

This program funds ombudsman and one-on-one beneficiary counseling services for states operating Financial Alignment Model Demonstrations—integrated care programs serving Medicare-Medicaid dual-eligible individuals. Funded activities include person-centered assistance to beneficiaries, caregivers, and authorized representatives in resolving insurance-related problems, selecting health plans, understanding consumer rights, and accessing benefits within the demonstration models. Recent awards supported ombudsman programs in Rhode Island, South Carolina, Massachusetts, and California, as well as counseling initiatives in Michigan and Massachusetts. Typical services span complaint investigation, outreach and education, linguistically and culturally competent support delivered via phone, email, website, and in-person contact, and advocacy regarding plan compliance and beneficiary protections.

The program portfolio consists entirely of 29 Cooperative Agreements totaling $44.1 million in federal dollars obligated over the past thirteen years, with recent activity concentrated between 2015 and 2020. CMS funded all awards; the portfolio shows a median award magnitude of approximately $1.5 million to $2 million per cooperative agreement. Award volume peaked in 2017 at seven awards; the most recent awards in August and November 2020 ranged from roughly $1.1 million to $2.9 million. Eligible applicants are limited to state agencies and entities designated by states that have signed memoranda of understanding with CMS to implement a Financial Alignment Model Demonstration. Project periods are 45 months, with funds released annually. Award recipients must maintain accounting and programmatic records for three years.

This program is not actively soliciting. The last funding opportunity closed in August 2020, and the source explicitly states there are no future deadlines. All awards to date have concluded or are nearing completion, with most final performance dates in 2023–2025. Organizations should monitor CMS announcements for potential future solicitations if their state operates an active Financial Alignment Model Demonstration.
