About this program

The Health Resources and Services Administration (HRSA) within the Department of Health and Human Services operates the Community Integrated Service Systems program (CFDA 93.491) to fund states and communities building comprehensive, integrated systems of care that improve access and health outcomes for all children, including those with special health care needs, with a currently open opportunity due July 10, 2026.

HRSA awards competitive grants for up to 2 to 5 years, subject to fund availability, with potential for noncompetitive continuation awards contingent on progress reporting and federal determination of ongoing benefit. Final award decisions are made 4 to 6 months after application receipt following objective review committee recommendation and delegated HRSA program official approval. Award preference is provided to high infant mortality areas for specific investments including maternal and infant health home visiting programs, projects increasing obstetrician and pediatrician participation, integrated maternal and child health service delivery systems, maternal and child health centers, and rural maternal and child health projects.

The program supports evidence-based strategies improving prenatal-to-age-5 family access to quality health care, early screening for physical and mental health needs, and connections to family support services. The Early Childhood Comprehensive Systems SEED Project (ECCS SEED, HRSA-26-057), posted December 12, 2025, with application due July 10, 2026, is the current open opportunity under this program. It operates as a Cooperative Agreement and is open to state governments, county governments, city and township governments, tribal governments, tribal organizations, special district governments, higher education institutions, nonprofits including non-501(c)(3) organizations, small businesses, for-profit organizations, and independent school districts.

The source provides no recent grant award portfolio data, award counts, funding concentration figures, or representative recent award examples. Final decisions on submitted applications arrive 4 to 6 months post-receipt. Recipients must maintain grant accounting records for 3 years after federal financial report submission; entities expending $1 million or more in federal awards annually are subject to single or program-specific audit requirements under 2 CFR 200, Subpart F.
