About this program

The Centers for Medicare and Medicaid Services (CMS) Innovation Center is awarding cooperative agreements to support the Accelerating State Pediatric Innovation Readiness and Effectiveness (ASPIRE) Model, with applications due by August 31, 2027, CFDA 93.487.

The ASPIRE Model tests aligned care delivery and value-based payment approaches designed to improve outcomes for children and youth in Medicaid and CHIP who have or are at risk of developing complex medical, behavioral, or developmental needs. Funded entities—State Medicaid Agencies collaborating with accountable entities such as pediatric accountable care organizations or managed care entities—will deliver enhanced care coordination and transitional care. The model will operate in up to five states and measure success against four primary aims: improving quality, reducing avoidable utilization, strengthening family experience, and supporting enhanced care coordination.

CMS will award a maximum of five cooperative agreements, each for a combined nine years and eight months. The funding period consists of a twenty-month Pre-Implementation Period (January 2028 through December 2029) followed by an eight-year Implementation Period (January 2030 through December 2037). Recipients draw down funds as needed through the HHS Payment Management System.

An optional Letter of Intent may be submitted by June 18, 2027. Merit review will evaluate applicant readiness to conduct required work, projected impact scope, programmatic and grants management compliance, cost reasonableness, and likelihood of achieving expected benefits. CMS will notify award recipients in writing via Notice of Award.

Organizations with annual federal expenditure of one million dollars or more are subject to single or program-specific audit requirements under 2 CFR 200, Subpart F. Non-federal entities spending less than one million dollars annually in federal awards are exempt from federal audit requirements for that year.
