About this program

The Department of Health and Human Services Indian Health Service funds competitive cooperative agreements and project grants to implement and evaluate diabetes prevention or cardiovascular disease prevention programs serving American Indian communities under CFDA 93.442, with the most recent grant awards issued on September 30, 2013.

This program has not issued new awards since 2013, making it inactive for grant-seeking purposes. The portfolio contains 82 total awards spanning from 2004 through 2013, with award activity concentrated in 2004 (50 awards) and 2010 (18 awards), followed by a sharp decline to 1 award in 2012 and 13 awards in 2013. No recent grant opportunities are currently posted.

The program funds demonstration projects in two specific areas: primary prevention of diabetes in American Indian populations, and prevention of cardiovascular disease in people with diabetes. Representative awards include the Sisseton Wahpeton Oyate Diabetes Prevention Program ($685 thousand), the Lake County Tribal Health Consortium Diabetes Prevention Program ($1.575 million), the Ketchikan Indian Community Diabetes Prevention Program ($1.622 million), and the Cherokee Nation Diabetes Prevention Program ($1.984 million). Projects typically ran for three to four years with completion dates in the 2015–2017 period.

The assistance-type mix is predominantly cooperative agreements, which account for 77 percent of awards (63 agreements), with project grants representing the remaining 23 percent (19 awards). All funding comes from the Department of Health and Human Services Indian Health Service. Combined federal dollars obligated total $99.2 million across all 82 awards. Individual awards in recent funding cycles (2010 and 2013) typically fell in the $972 thousand to $1.622 million range, though some project grants were considerably smaller. The program shows no current activity and no open solicitations.
