About this opportunity

The Virginia Department of Health is requesting applications for the Innovative Maternal Care sub-initiative under the Virginia Rural Health Transformation Program, a federally funded initiative administered by the Centers for Medicare & Medicaid Services, with applications due October 6, 2026 at 5:00 p.m. EST via the eVA procurement portal and a pre-application conference scheduled for September 16, 2026 at 2:00 p.m. EST via WebEx.

Eligible applicants include licensed EMS agencies, nonprofit organizations, state and local governmental entities, hospitals and health systems, Federally Qualified Health Centers, rural health clinics, primary care clinics, and institutions of higher education. All applicants must maintain active SAM.gov registration and demonstrate organizational capacity to manage federal subaward funds in compliance with 2 CFR Part 200. VDH seeks community-driven maternal health projects that expand access to prenatal, perinatal, postpartum, and maternal behavioral health services in Federal Office of Rural Health Policy-designated rural Virginia communities across the state's seven RHTP regions, with a goal of making at least one award within each region subject to application quality and funding availability.

Approximately $11.6 million in total federal funding is anticipated to be awarded during the initial performance period, with individual awards ranging from $50,000 to $1,000,000 based on the scope and scale of proposed projects. No cost sharing or matching is required. The initial subaward period runs from execution through September 30, 2027, with potential continuation funding available through September 30, 2030 subject to satisfactory performance and available funding. Administrative costs are capped at 10 percent of total award amounts, and VDH anticipates minimal funding availability for direct administrative costs and no availability for indirect costs. Equipment purchases exceeding $10,000 per unit and capital expenditures are capped at 20 percent of total CMS award amounts; construction and building expansions are prohibited. All subrecipients must submit quarterly and annual programmatic and financial reports, participate in VDH technical assistance and learning collaborative activities, maintain all grant-related records for a minimum of five years, and acknowledge CMS funding in all publications describing funded projects. Questions are due September 25, 2026, and VDH anticipates issuing award notifications on or around November 13, 2026.
