About this opportunity

The Pennsylvania Department of Health's Long-Term Care Transformation Office is requesting applications for the Long-Term Care Quality Investment Program (RFA 67-212), a grant initiative distributing approximately $12 million in fixed awards to eligible long-term care facilities, with applications due October 1, 2026 at 7:00 a.m. ET. The pre-application conference held September 9, 2026 was strongly encouraged as an evaluation component, and questions were due September 16, 2026 by 12:00 p.m. ET.

Eligible applicants are Skilled Nursing Facilities, Personal Care Homes, Assisted Living Facilities, and Intermediate Care Facilities operating in Pennsylvania. Applicants must be registered as active Commonwealth vendors in SAP, maintain a clean record in the Contractor Responsibility Program System, and hold a valid Unique Entity Identifier (UEI) from SAM.gov. Facilities housing multiple levels of care must submit separate applications for each level. A minimum of 50 percent of total funds is reserved for SNFs.

The program funds two categories across six tracks: workforce development (leadership training, clinical and non-clinical skills training, and nursing assistant training at $5,000–$5,500 per person) and infrastructure improvements (technology, electronic health records, and resident support initiatives ranging from $10,000 to $125,000 per track). Maximum funding is capped at $2,500 per bed or $250,000 per facility, whichever is less.

Evaluation criteria include Social Vulnerability Index by county and zip code, Medicaid and SSI utilization rates, geographic location within Medically Underserved Areas or Health Professional Shortage Areas designations, alignment of proposed interventions with facility needs, and pre-application conference attendance. The Department anticipated initially awarding between 48 and 100 agreements. The initial performance period runs January 1, 2027 through September 30, 2027, with potential extension of up to 90 days by written notice or longer through formal amendment. Award recipients must submit mid-term progress reports within 180 days of execution and final reports within 30 days of grant expiration. Addendum 1 was posted September 1, 2026.
