Required compliance documentation includes an ACORD Certificate or Certificate of Liability Insurance with endorsements and stop gap coverage, signed verification of current address and contact information, a completed Contractor/Subcontractor Affirmation and Disclosure Form, an Affirmative Action Program Verification Form, Secretary of State Charter/Registration Number, current Bureau of Workers' Compensation certification, and any applicable MBE, EDGE, VBE, WBE, or specialty trade certifications.
