Added: May 11, 2017 9:21 am This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0418 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 339112 Surgical and Medical Instrument Manufacturing and small business size standard is 1,000 employees. This is a total service disabled veteran owned small business set-aside. The requirement is for the REM B Power System and accessories. The Rem B Power System will be delivered to the Louis Stokes VA Medical, 10701 East Boulevard, Cleveland OH 44106; this will also be the FOB destination.

This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95. We require REM B Handpieces as a Name Brand only, since they are the only units that can plug into our existing Stryker CORE2 electronic drill consoles. We currently have one console in each operating room and they are utilized by numerous services within the Operating Room and Ambulatory Surgery.





Performance Monitoring:

The COR will work with the warehouse staff and biomedical engineering to ensure delivery and incoming inspection to determine for fitness for use.



Security Requirements:

The contractor will not have access to VA sensitive information.

Risk Control:

The Surgical Service will follow any safety recommendations given by the vendor for the equipment.
