Amendment 0001:
The purpose of the amendment is to replace Attachment 1, Performance Work Statement, dated 27 Jun 2018, with revised PWS, dated 8 Aug 2018. Changes to the PWS are marked in red text.

Offerors are to use the conformed copy of the solicitation document (FA4610-18-Q-0004-0001) provided with this notice to submit their Part I, Price Proposal.


Offerors are to fill in Block 8 and Blocks 15a, 15b, and 15c of SF30 Solicitation Amendment and return with their Part I, Price Proposal.


The proposal due date is not extended.


Questions and Answers, dated 13 Aug 2018, are posted with this amendment.

This is a combined synopsis/solicitation for commercial services prepared in accordance with the guidance in FAR 12.6, as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-101 and DFARS DPN 20180629.

This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. This combined synopsis / solicitation is issued as Request for Proposal (RFP) FA4610-18-Q-0004.


This acquisition will be competed as a Full and Open Competition under North American Industry Classification (NAICS) Code 621910, "Ambulance Services", for which the size standard is $15.0M.


This combined synopsis/solicitation is expected to result in award of a firm fixed-price contract for Patient Transport Services for Vandenberg AFB, CA, as described in Attachment 1, Performance Work Statement (PWS). The period of performance includes a 7-month basic period from 01 Oct 2018 through 30 Apr 2019, three (3) one-year option periods extending from 01 May 2019 through 30 Apr 2022, and an optional six (6) month extension period through 31 Oct 2022. All option periods to be exercised at the unilateral right of the Government.


Offerors must submit written proposals using the SF1449 Solicitation/Contract/Order for Commercial Items form (SF 1449) provided with this announcement and must follow the instructions contained therein. Offerors are strongly advised to read and follow the Addendum to FAR 52.212-1, Instruction to Offerors, and the Addendum to FAR 52.212-2, Evaluation.


All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov). Offerors are urged to complete or update SAM registration as soon as possible as new procedures could delay an offeror's eligibility for award. As of 11 Jun 2018, all entities registering in SAM must submit a notarized letter appointing their authorized Entity Administrator. Content of the letter and mailing address may be found on the sam.gov website. Entities registered in SAM are advised to log into SAM and review their registration information, particularly their financial information.
