About this file

This document is a Worker Protection and Investment Certification Form (BOP 2201) issued by the Commonwealth of Pennsylvania, mandated by Executive Order 2021-06. The form requires contractors and grantees to certify compliance with multiple Pennsylvania state labor and workforce safety laws, including acts related to workplace misclassification, employment of minors, minimum wage, prevailing wage, equal pay, and various industry-specific labor protections.

The certification carries significant legal implications, with potential penalties for false declarations including contract termination and a three-year prohibition from bidding on state contracts. Contractors must affirm their compliance with the listed labor laws and commit to immediately reporting any changes in their compliance status. The form requires a signature from an authorized official who understands that providing false information could result in legal consequences under Pennsylvania's unsworn falsification statutes.
