About this file
This is a Price Form attachment for an Invitation for Bid (IFB) issued by the Maryland Department of Transportation State Highway Administration (MDOT/SHA) for pest control services in Worcester County. The document establishes the pricing structure and submission requirements for vendors bidding on pest control services at MDOT/SHA facilities located in Worcester County, Maryland. Bidders are required to submit detailed pricing for pest control services, including treatment frequency, types of pests to be controlled, and any associated labor and materials costs. The price form serves as the standardized template through which all bidders must present their cost proposals, ensuring uniform comparison of bids across all responding vendors.
The price form includes line items for various pest control service categories and pricing columns for bidders to complete with unit costs and total amounts. Bidders must complete all required pricing fields, including labor rates, material costs, and any ancillary service charges, with pricing typically submitted on a per-visit, per-treatment, or monthly service basis depending on the service category. The form establishes the framework for cost evaluation and contract award determination, with all pricing submissions subject to competitive review against established evaluation criteria.
