3. Description of Supplies/Services: To Provide Annual Accreditation and Application Fees for four existing Residency Programs and additional programs as mandated by the Surgeon General.

The Accreditation Council for Graduate Medical Education (ACGME) Annual Accreditation is mandated too train physicians in a variety of specialty programs in support of the Army’s mission. In order to conduct the programs all must meet the accreditation standards and undergo a site review in order to be accredited through the ACGME. This service provides continued accreditation for existing Residency Programs (Family Medicine Residency Program, Obstetrics/Gynecology Residency Program) and two projected Residency Programs, Transition Year Residency Program and Internal Medicine Residency Program) in FY19. This service also provides for continued accreditation of the outyears for four programs in FY20 (Family Medicine Residency Program, Obstetrics/Gynecology Residency Program, Transition Year and Internal Medicine Residency Program) and two projected Residency Programs Orthopedic Residency Program and General Surgery Residency Program in FY21, for a total of six. This service provides for continued accreditation of six programs in FYs 22-

23. Additionally, the application fees for two new programs in FY19 Orthopedic Residency Program and General Surgery Residency Program in FY19. The Residency Program Accreditation Fees and Application Fees have historically been accredited by the Accreditation Council for Graduate Medical Education (ACGME). The contract is for a period of five years (2019-2023) for a total cost of $209,200.00.

4. Authority Cited:

(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

(c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR

(d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR

(e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.

5. Reason for Authority Cited: This is a sole source requirement as the ACGME is the only organization that evaluates and provides accreditation to physician residency and fellowship programs in the United States. Womack Army Medical Center, is designated by the Army of the Surgeon General, is responsible for conducting residency and fellowship programs in order to train physicians in a variety of specialty programs in support of the Army’s mission. In order to conduct a residency or fellowship program, all of the programs must meet the accreditation standards and undergo a site review in order to be accredited through the ACGME. WAMC has existing residents in various program year groups that an accreditation by ACGME has already covered. All Army residency and fellowship programs are accredited by ACGME. Completion of an ACGME residency or fellowship allows the physician to sit for and obtain specialty board certification. The medical center as a whole benefits from continued accreditation because WAMC is able to train military physicians in support of the Army’s mission.

6. Actions to Increase Competition: If in the future other accreditation services become available, research will be performed to consider other products that meet or exceed the current training requirements. All reasonable resources were used to verify and confirm that this is the only sole source for this service. The ACGME is the only organization that evaluates and provides accreditation to physician residency and fellowship programs in the United States.

7. Market Survey: Market research revealed that the Accreditation Council for Graduate Medical Education (ACGME) is the only organization that can provide the services needed. Internet searches for accrediting bodies for allopathic physician residencies (the Accreditation Council for Graduate Medical Education-ACGME) and osteopathic physician residencies (the American Osteopathic Association-AOA) in the United States. The tri-services (Army, Navy, Air Force) only support allopathic physician residencies (ACGME accredited). On 26 February 2014, the ACGME, American Osteopathic Association (AOA), and American Association of Colleges of Osteopathic Medicine (AACOM) announced their agreement to a Memorandum of Understanding (MOU) outlining a single graduate medical education accreditation system in the United States. The single GME accreditation system allows graduates of allopathic and osteopathic medical schools to complete their residency and/or fellowship education in ACGME-accredited programs and demonstrate achievement of common milestones and competencies. In 2015, the AOA merged with the ACGME and now there is only one council that performs this accreditation function for military treatment facilities funded by the military. There are other companies that have equivalent solutions that provide accreditation for programs internationally and in Canada, but do not meet the specific needs of this medical center.
